As stated in our letter of May, 11, 2015,your exterior rear and front egress
deck/stairs are in a dangerous state of disrepair in violation of the State Building
Code. The disrepair has become worse since this letter and poses a threat to the
public since the issuance of referenced letter. Under Section 116.0 of the
Massachusetts State Building Code (780 CMR), you are required to begin to
make this structure safe within 24 hours of receipt of this notice.
You are hereby directed to take corrective measures to make these stairways safe
and have them approved by our Department by 12:00 p.m., Tuesday June 30,
2015.
Failure to address this situation may result in further action by the City of Salem,
which can include this department causing the issuing of tickets or the possible
removal of this unsafe structure, under the provisions of 780 CMR Section 121,
followed by an attachment of liens to the appropriate properties to cause repayment of
any incurred expenses.

At a recent inspection of the above property,I observed that the exterior rear and
front egress deck/stairs are in a dangerous state of disrepair in violation of the State
Building Code. Additionally smoke detectors and emergency light in the common
areas are not functioning. This life safety equipment must be repaired immediately
upon receipt of this letter.
The stair needs to be repaired or replaced immediately. You are hereby directed to
enlist the assistance or a Registered Architect or Professional Engineer to evaluate
the existing structure and indicate necessary repairs or to design a code compliant
stair.
You are also hereby directed to address this violation within 21 days of this notice.
Failure to do so may result in further actions being brought against you,up to and
including fines and the filing of criminal complaints at district Court. You have the
right to appeal this decision to the Bureau of Building Regulations and Standards,
One Ashburton Place,Boston, Massachusetts.

193 1 /2 North Street . She informs me that Michael F . Malionek , the
owner of property adjacent to hers at 193 North Street , has added
improvements to the third floor of that building and is renting the
same in violation of the City ' s Zoning Ordinance . The property is
located in a R-2 District .
Mrs . Young also informs me that Mr . Malionek has constructed an
accessory building , namely a shed , on the property which appears to
be in violation of Section VII ( h) of the Salem Zoning Ordinance .
The property on which the shed was erected Is a corner lot and the
Ordinance states as follows : "Accessory buildings and structures
such as garage and tool shed shall be subject to the following
regulation : ( 1 ) No accessory building or structure shall be located
within any required front yard or within any side yard of a corner
lot . "
I enclose herewith a copy of Mr . Mallonek ' s Deed which clearly
shows that the property Is located at the corner of North Street and
Cressey Avenue and is , therefore, in violation of the Zoning
Ordinance .

I reside at 193 North Street, Salem. I have good reason to believe
that the abutting property which is numbered 193 North Street, which has
always been a two-family dwelling house is being converted into a three-
family dwelling house.
This belief is based on the fact that during the past few. weeks, I
have seen building materials being brought into the building and can hear
construction taking place on the third floor.
I would appreciate your making an investigation of this matter since
it would appear that this is a violation of the Zoning Regulations. I
would appreciate a response to this matter.

Dear Sir•
I reside at 193 North Street, Salem. I have good reason to believe
that the abutting property which is numbered 193 North Street, which has
always been a two-family dwelling house is being converted into a three-
family dwelling house.
This belief is based on the fact that during the past few weeks, I
have seen building materials being brought into the building and can hear
construction taking place on the third floor.
I would appreciate your making an investigation of this matter since
it would appear that this is aviolation of the Zoning Regulations. I
would appreciate a response to this matter.
