Dear Owner
Due to recent complaints, and from correspondence dating back to 2005 a site visit was done on your
property on October 30"2017. We found building code violations as described below.
First The main house has had multiple tarps installed on the roof deck. State Building Code 780 CMR
Chapter 9 section R903 Article R903.1 states roof decks shall be covered with approved roof covering
secured to the building. Plastic tarps are not considered an approved roof covering.
Second you were ordered to clean up the debris from a shed fire dating back to September 2017.
The old shed floor still remains . I am under the impression that you want to rebuild this shed . Due
to the Salem Zoning Ordinance only one shed is allowed per lot and another shed exists on the property
This remaining shed also has tarps on the roof and needs to be changed to an approved roof covering as
Mentioned above. The shed floor needs to be disposed of properly
To avoid legal action please follow instructions as described
1. A permit needs to be pulled by November 17 2017 for both roofs
2. Work needs to begin by December 12017 for both roofs
3. All work described above must be completed by January 12018

Re: Shed Debris Due To Fire
Dear Ownerj
In reference to the previous letter sent out giving you until October 2 2017
to clean up all debris at 177 North Street due to a shed fire. The time has expired
to clean up any and all debris caused by the shed fire. Enclosed is a fine in accordance
with the city Ordinance of unsightly conditions SCO 24-12.1. Please be advised that
you will be fined every 3 days until all debris is removed from the property and disposed
of properly. Note putting debris in boxes on a city sidewalk with a sign that says free
is not considered disposing properly.

Re: Shed Debris Due To Fire
Dear Owner;
This department has received numerous complaints regarding debris on property
Due to a shed fire on your property at 177 North Street. During a recent site visit
on September 12 2017 you told me that two weeks would be a sufficient time for you
to clean up any and all debris due to the shed fire. I am giving you until October 2
2017 to completely clean up any and all debris due to shed fire. If all debris is not cleaned
up and disposed of properly it will result in fines from City Of Salem . Thank you for
your cooperation in this matter.

This Department responded to a request from Salem P.D. to investigate the Hazards
discovered while responding to Medical Call at your building.
The interior of the Building is in various states of both disrepair and construction. There
are large sections of the walls and ceilings that are open creating places for a Fire to
spread rapidly.Also observed were exposed wires.
The interior is loaded with various Household goods and miscellaneous building
materials.The exterior is open to the weather at all eaves as well as several areas of
sidingThe large quanity of materials and the maze they cause, combined with the many
code problems. Cause me to Declare this building an "Unsafe Structure"per State
Building Code 780C.M.R. section 121 throughl21.8.The yard also contains a large
volume of debris and other combustibles further endangering First Responders.
Per the State Building Code, you have until 12:00 May 29 to secure the building.
The Bing is not to be occupied until a certificate of Occupancy is issued by this
