In short, I believe Mr. McNeal is correct in his interpretation of the ordinance. So long as placed on a pre- existing nonresidential structure or municipal structure, a WCF applicant only needs a building permit from the building inspector once meets other conditions (ie. not more than 15 feet above building). Brennan, Chief Salem Fire Department Sirs Following is a report of conditions found to exist at the new portion of the Holyoke Insurance Co. office building as a result of several inspections conducted within the past ten days. 1. Protection guard rail has been provided as requested for air handling unit located in lower level garage. The unit located on the first floor level still requires protection. 2. It is to be noted that all sprinkler heads located in the garage levels are still covered with tape , because the painter is not throub i with his eel .ing This portion of the work should be completed as soon as possible as the heads are not in proper condition to serve in case of emergency. This is especially important in the lowere level garage presently occupied under a temporary occupancy permit. 3. Certification of the carpeting installed on all office floor areas has not been received by this office. We have no flame spread or smoke contribution ratings. 4, Permanant emergency lighting system was tested on April 17, and the emergency generator was found to operate properly. The cafeteria area was found to have no emergency lighting and should be provided with same. 5. Several fire extinguishers are not in place as of this date. Primarily the extinguishers recommended for the Hose cabinets. 6. It is to be noted that several cabinet doors for extinguishers and fire hose are sticking, and sagging. 7. Several vertical and horizontal. pipe holes were found through out the structure and shall be completely filled in to prevent spread of fire. 8. Self closing device shall be required on corridor door from fourth floor office area and an exit sign shall be properly positioned. Page #2-Holyoke Ins. Co. New Office Building (Continued) 9. Sprinkler test was conducted on April 14, 1975 by this office. Results are as followss a. The hydrostatic test was conducted on April 9. 1975 with 215 P. S. I. on the system for two hours and found satisfac- tory. b. The Test on the entire system with our apparatus was conducted using Engine #3, Engine #4 and Ladder #1. on April 14, 1975. c. Readings taken are as follows; at street pressure. 1. Gauge on street side of O. S. &Y valve read 80 P. S. I. (Static ) 2. Gauge on system side of O. S. &Y, valve read 85 P. S.,I. (Static ) 3. Gauge on fourth floor stairwell read 65 P. S. I. (Static ) 4. Gauge on lower lever off 100 feet of 12" hose read 71 P. S. I. (Residual) 5. Gauge on fourth floor off 100 feet ofl2" hose read 56 P. S: I. (Residual) d. Readings taken are as follows; Using Engine #3 pumping at 100 P. S. I. into F. D. Connection, with one 22" line. 1. Gauge at 4th floor, off 100 feet of 12" hose; 81 P, S. I. (Static ) and 71 P. S. I. (Residual) 2. Gauge at lowere level, off 100 feet of 12" hose; 92 P. S. I. (Static) and 78 P. S. I. (Residual) e. Readings taken are as follows; Using Engine #3 pumping at 150 P. S. I. with two 22" lines into F. D. Connection. 1. Gauge at 4th floor, off 100 feet of 12" hose; 131 P S. I. (Static) and 115 P. S. I. (Residual) 2. Gauge at Lowere level, off 100 feet of 12" hose; 150 P. S. I. (Static) and 130 P. S. I. (Residual) f. System was found satisfactory for use by the Salem Fire Dept. 10. The box provided for spare heads located adjacent to the O. S. & Y. valve has no spare heads as required by law. This box shall also be properly labeled. 11. Fire alarm system plans were never submitted to this office for stamped approval. The following items were noted as not proper nor acceptable to this office. a. The Salem Fire Prevention Bureau does not accept bells as fire alarm sounding devices. Horns are the acceptable means for fire alarm evacuation and all bells shall be re- placed with properly installed horns. It shall be noted that no provision has been made for lights on the fire alarm system as required for handicapped areas. b. Certain manual fire alarm pull stations are located at the level of the electrical switches and should be placed at eye level, as required by this office, so as to comply with Section 3-1. 1. 2 of N. F. P. A. Standard # 71 and not be confused with an electrical light switch. c. It is recommended that the present office building be pro- vided with horns for the fire alrm system so as to provide a standard notification in the event of a fire. d. This office shall be present for a test of all newly in- stalled fire alarm systems as provided for in the Salem Fire Code. Page #3-Holyoke Ins. Co. New Office Building (Continued) 12, All other conditions were found satisfactory for occupancy at the time of inspection, subject to cleaning up of excessive e buildup of combustible rubbish in first floor level garage. 4 13. It is to be noted that the Fire Department connection located on Norman Street, is still in a temporary condition. The plate } required has not been placed on the temporary piping. The }} height of the Fire Department connection appears to be only F about 13 inches from the finished grade of this area and is t in violation of Section 1206. 71 of the Massachusetts Statc Building Code which requires a minimum height of two feet above grade. This condition will apparently be a problem to correct at this time, and a deviation will be permitted by this office, if acceptable with the Building Inspector, so as not to create an extreme hardship. This condition shall be noted by the High Point Sprinkler Co. and they are hereby notified that no deviation will be permitted on any furthur installations by this company, 14.
