International Residential Code 2009
R105.1 Required.Any owner or authorized agent who intends to construct, enlarge,
alter, repair, move, demolish or change the occupancy of a building or structure, or to
erect, install, enlarge, alter, repair, remove, convert or replace any electrical, gas,
mechanical or plumbing system, the installation ofwhich is regulated by this code, or to

Department has been satisfied that proper construction permits have been applied for, and
issued to, appropriately licensed personnel. No work may be undertaken without written
approval from the Salem Building Department.

The above listed property has been found to be in violation of the following State Codes
and/or City Ordinances:
An illegal addition has been built on top of an existing deck without the
required permitting. 780 CMR, State Building Code, Section 18.1, states that
no person shall change, alter, or change use of a structure. The penalty for
violating this provision $1000 per day with each day being a separate violation.
Due to a complaint of an illegal basement apartment this office is requiring an
inspection of this property under provisions of 780 CMR, State Building Code,
Section 115. This inspection will require access to all areas within this
structure and will be conducted by a combined inspectional team including, but
not limited to, Fire, Health, Building Inspectors.
Said violations must begin to be corrected, repaired, and/or brought into compliance
within 2 days of your receipt of this notice. Failure to do so may result in further actions
being brought against you, up to and including the filing of complaints at District Court.

durable, rodent-proof material. Rubbish shall be stored in receptacles of metal or
other durable, rodent-proof material. Garbage and rubbish shall be put out for
collection no earlier than the day of collection.

used as s liner in watertight receptacles with tight-fitting covers as required in 105

premises of the owner not later than 7:00 a.m. on the day of collection and not
before 6:00 p.m. on the day preceding the day of collection, and shall be removed
from the sidewalk on the same day as emptied. No commercial establishment shall
place or cause to be placed more than four barrels or other containers of ordinary
..commercial wastes or any extraordinary commercial or industrial wastes or tree
waste upon any aldeyalk or way for disposal.
Should you be aggrieved by this Order, you have a right to request a hearing before
the Board of Health. A request for a Hearing must be received in writing in the
office of the Board of Health within seven (7) days of receipt of this Order. At
this Hearing, you will be given an opportunity to be heard and to present vltness

Acting on complaints to this office from the Board of Health on March 31 ,
1989 of possible code violations. During the inspection of the above referenced
property we found you in violation of the Mass State Building Code section 113,
and 121 (working with permits) .
Anyone who shall violate a provision of this code shall be punishable by a
fine of not more than one thousand dollars ($1 ,000. ) or by imprisonment for not
more than one year, or both for each violation. Each day during which any
portion of a violation continues shall constitute a separate offense.
