Dear Mr. Maurice;
The above listed property has been posted with a Stop Work Order due to being in
violation of the following State Codes and/or City Ordinances.
780 CMR Massachusetts State Building Code, Section 118.1, regarding violations of
the construction code, states that it is unlawful to add, alter, or construct any structure
without the proper permit to do so.
No further work may be done until such time as the order is lifted. Any person who shall
continue any work in or about the building or structure after having been served with a
Stop Work Order, except such work as that person is directed to perforin to remove a violation
or unsafe condition, shall be liable to a fine of not more than $1000, or by imprisonment for
not more than one year, or both for each violation; with each day constituting a separate
violation.
If you feel aggrieved by this Order, your right of appeal of this action would be through the
Board of Building Regulations and Standards, located at Room 1301, 1 Ashburton Place, Boston.

In accordance with Chapter 111, Sections 127A and 127B, of the Massachusetts
General Laws, 105 CMR 400.000: State Sanitary Code, Chapter 1: General
Administrative Procedures and 105 CMR 410.000: State Sanitary Code, Chapter
II: Minimum Standards of_Fitness-for-Human_Habitation, an inspection was
made of your property at 107 Mason St. `► Salem, Massachusetts,
occupied. by Mrs. Lopez`__ This inspection was
conducted by V. Moustakis A. Alcontara Salem Health Department, on
8/21/85 3:00 p.m.
Based on said inspection, you are hereby ordered to take the following
action within 24 hours of receipt of this order:
Repair and/or replace fluorescent light in kitchen
that is not working
Investigate and repair leak from bathroom sink
Replace missing window and frame in living room
Replace missing window pane in living room, and repair
all windows to fit properly, and be weathertight
Replace broken storm window (bedroom off living room)
Replace storm window (bedroom to left of kitchen)
Replace window (bedroom to right of kitchen)
Secure wire handing down front of house (exterior)
Provide lighting for back hall
Repair all three back porches so that ballusters (or boards) are
placed so that a 6 inch sphere cannot pass thru. CONTACT
BUILDING INSPECTOR IMMEDIATELY
Replace missing floorboards on first floor back porch and repair
floorboards that are not secure and feel ready to break or split

Based on this inspection, you are hereby ordered to take the following
action within 5 days of receipt of this order:
Repair hole in window sill (bedroom off living room)
Replace missing sashcords (same room as above)
Replace missing sashcords (bedroom to left of kitchen) -
Patch large hole in ceiling (bedroom to left of kitchen)
Provide light fixture, repair ceiling and provide screen
(bedroom .to right of kitchen)
Provide adequate hardwired detectors for hallways (CONTACT
FIRE DEPARTMENT IMMEDIATELY) and provide adequate detectors
throughout apartments all 3 floors
Post name, address and telephone of owner and/or agent
Remove old inground garbage containers from exterior
(back yard) that is trip hazard
Based on said inspection, you are hereby ordered to take the following action
within 30 days of receipt of this order:
Provide adequate locking mechanisms for all windows needing
them
Repair wallpaper falling from wall in back hall
Repair ceiling in first' floor front hall

SAFETY AND WELL-BEING OF THE OCCUPANTS.
Failure on your part to comply within the specified time will result in a complaint
being sought against you in Salem District Court.
Should you be aggrieved by this Order, you have the right to request a hearing before
tke Board of Health. A request for said hearing must be received in writing in the
office of the Board of Health within seven (7) days of receipt of this Order. At
said hearing, you will be given an opportunity to be heard and to present witness
and documentary evidence as to why this Order should be modified or withdrawn. You
may be represented by an attorney. Please also be informed that you have the right
to inspect and obtain copies of all relevant inspection or investigation reports,
orders and other documentary information in the possession of this Board, and that
any adverse party has the right to be present at the hearing.
Please be advised that the conditions noted may enable the occupant(s) to use one
or more of the statutory remedies available to them as outlined in the enclosed
inspection report form.
