The following items shall be noted as requireing compliance for approval of the project by the Salem Fire Department. (Authority of Section 1200. 1. 1 fuss. Bldg. Code) 1. Fire Department Access: a. Access for fire apparatus is not acceptable as shown on the plan. 1. Building #1 is approx. 180 feet from a point where fire apparatus would be situated for first line duty. 2. Building #2 is approx. 100 feet from a point where fire apparatus would be situated for first line duty. 3. Buildings #4, #59 #79 #8, #13, #14 and #20 are all approx. 60 feet to 75 feet from a point hwere fire apparatus would be situated for first line duty. b. As a result any structure situated furthur than fifty ( 50 ) feet from a point of access for fire apparatus, shall require an approved pathway (paved surface) to be provided , whereby said fire apparatus can easily be manuvered into position for stretching of pre-connected fire hose, for service in the structure. The point of access from the roadway to the pathway, shall be pro- vided with a curb cut and entry at least fifteen ( 1.5) feet wide. The approved pathway shall be at least ten ( 10) feet wide. 2. Water Main= a. No water main has been indicated on the plan submitted. b. The water main to be provided and approved , shall be sized as determined by the Director of Public Services and the Fire Farshal.. c. The water main shall be designed to provide for a loop with the the existing adjacent water mains. One connection shall be made to the end of the existing water main on Maple Ave, and another connection to Peter Road area. d. The resulting loop would provide for adequate pressure and volume. 3. Post Hydrants& a. All post hydrants shall be compatable to the type presently used by the City of Salem and approved by the Director of Public Services. b. The hydrant shown on the plan, situated at the first parking area, (near Bldg. #3 ) appears satisfactory. c. Additional pogt hydrants shall be located as follows: 1. Adjacent to pump 'stati$n 2. No structure shall be started until a post hydrant has been installed , tested and placed in service, within five Hundred ( 500) feet of said construction. 4. Revised Plans a. A revised plan acceptable to the Director of Public Services and the Head of the Salem Fire Department, shall. be submitted for stamped approval, prior to issuance of a building permit. 5. Structure Plans: a. Each structure shall require a Form #81 (Plan approval application) to be submitted prior to issuance of a building permit. b. Said plans shall indicate the type and location of required smoke detectors. c. Three sets of structural plans are required for each type of structure, for stamped approval. One set will be retained by the Salem Fire Department. The remaining sets shall be submitted to the building inspector, with an individual form #819 for application for a building permit. Each Form //81 shall indicate a street address for individual structures. This street address can be obtained from the City of Salem Assessors office. d. Upon completion of each structure, the contractor shall call for a final inspection. At the time of final inspection the electrical contractor shall make application on Forms obtained from the Salem Fire Prevention Bureau, for approval of the fire detection devices and a Certificate of Compliance shall be issued for each structure. (Form #82) e. Upon sale of each individual condominium, a separate Certificate of Compliance shall be required for each unit. (Form 81A) 6. Provisions shall be made for one Street Master Fire Alarm Box, on this sub-division. Location to be determined by the Salem City Electrician and the Salem Fire Marshal. It shall be noted that as of this date, the plan is not acceptable to the Head of the Salem-Fire Department and shall require resubmission to obtain said approval. The site plan must include the items listed for water main layout and post hydrants, and provisions for adequate access by fire apparatus. The number of dwelling units shall be reduced to sixteen. The locations of these units and their respective lot lines shall be modified to reflect that plan submitted to the Planning Board on May 31, 1984, entitled, "Proposed Revisions to Tedesco Pond Place." All roadway specifications, open space areas, lot areas, and walkways shall be modified to reflect those changes shown on the May 31st referenece plan. . 2. All pedestrian walkways shall have a six inch gravel subbase and will be constructed of Yu" roofing gravel stone. The plan shall show the location of all walkways and shall define the accessibility of each walkway to the public. The plan shall show all walkways and driveways. The system of walkways, roadways, and driveways shall be approved by the Planning Board prior to the initiation of construction,,and the Planning Board shall reserve the right to require additional walkways, roadways and driveways as it deems appropriate. The adequate maintenance of all walkways, roadways and driveways in addition to the footbridge and settlement basin shall be sufficiently defined in the rules and regulations of the homeowners association and shall be subject to the approval of the Planning Board. 3. The list of requested waivers submitted by the developer on March 22, 1984, shall be modified to reflect the shortened length of the main roadway as seven hundred and sixty five feet with an access roadway, ten feet wide, for an additional one hundred ten feet. The hammerhead turnaround shalt be located at the northenmost corner of the site, adjacent to units twelve, thirteen, and fourteen. With the exception to those requested waivers, the plans shall conform to all requirements set forth in the Salem Massachusetts Subdivision Regulations, as amended December 4, 1975. The roadway right of way shall have a minimum width of thirty feet. The roadway shall have a minimum paved surface of twenty four feet and shall have a granite curb on both sides. Waivers shall be adopted with approval reservation by the Planning Board. A sidewalk shall be required on one side of the roadway, the location and composition of which subject to Planning. Board approval. 4. The developer shall resolve the issues addressed by the Acting City Engineer Richard Swenson, in the Definitive Plan and Wetlands Comments sheets dated April 17, 1984, and April 19, 1984, on file at the Salem Planning Department. Specifically: -2 84 JUL -2 P 1 :'51 a. The Engineer's concurrence with the plan is subject to the City Engineer's fapproval of the design, operation, maintenance, maintenance scheguJe•,install&tioa,TlCE and inspection of the sewage pump station. The installation of an eigl�r.inch i diameter ductile iron water main is required and subject to the City Engineer's. review and approval. b. There will be a minimum five foot horizontal separation between the force main and the gravity sewer main from the pump station to Elm Avenue. This is subject to the review and approval of the City Engineer. c. There will be no construction or alteration of the land upon existing drainage easements unless approved by the Planning Board. d. Since a portion of the sanitary sewer is to be constructed below water level, precautions shall be made to prevent water from entering the pipes during construction and to assure proper bedding forthe pipes. Specifically,.l) proper jointing of the pipes shall be adhered to; 2) proper care shall be assured in the laying of the pipe; 3) the pipe shall be sealed at the end of each construction day until the final connection and jointing has been completed; and 4) proper pumping procedure shall be followed to keep the trench de-watered during construction. These precautions are subject to the review and approval of the City Engineer and are to be in accordance with ASTM standards. e. Specific building locations and the base elevations of the lowest level shall be shown on the plans and shall be subject to the approval of the Planning Board prior to the start of construction of the development. 5. The development shall conform to the requirements set forth for Wetlands and for Cluster Residential Special Permits as defined in Section VII of the Salem Zoning Ordinance. 6. The five concerns raised by the Salem Board of Health in their letter dated April 9, 1984, (attached hereto) shall be adequately addressed to the satisfaction of the Planning Board. 7. Emergency access to Tedesco Pond Place from Eleanor Road shall be approved by the Fire Marshall and the City Engineer prior to the occupancy of any home. The homeowners association shall be responsible for the adequate maintenance of road emergency access and shall be responsible for snow removal at the terminus of Eleanor Road. A granite curb shall be installed at the terminus of Eleanor Road to impede general traffic and said curb shall be subject to the approval of the Fire Marshall and the City Engineer. The rules and regulations of the homeowners association shall specify that snow removal shall include that emergency access gate at Eleanor Road be kept free of snow at all times and that the road shall be accessible to emergency vehicles at all times. A breakaway barrier shail.be installed and maintained at the terminus of Eleanor Road and shall be subject to the review and approval of the Fire Marshall prior to installation. JUL / 8. This catch basin shall be constructed with an overflow mechanism. i c. The design of the storm water drainage system pertaining to run-off from Eleanor Road and Peter Road including the location of all catch basins shall be subject to the approval of the City Engineer. Construction shall not begin until the City Engineer has made a determination that the construction of the development shall have no detrimental effect upon the water drainage of the neighborhood. 9. Only calcium chloride may be used as a de-icing agent within the development. 10. Access to the development for construction purposes shall be from Tedesco Street only. 11. Common-areas and buffer zones shall be restricted by the following conditions: a. In the buffer zone adjacent to the existing residential neighborhood and in all common areas, the developer shall provide a landscaping plan which must be approved by the Planning Department prior to the sale of any tot in the subdivision. The developer shall request and take into account the thoughts and requests of property abutters.as the landscaping plan responsible for fully implementingthelm andsca loplan.TThe develohe per shall make certain that plantings shall be maintained in a healthy and thriving condition. As required by the Salem Massachusetts Subdivision Regulations, Section III, Part B, an Environmental Impact Statement must be submitted to the Planning Board. The developer must submit those items missing in the original submission. Specifically: a. Natural Environment: Items 1, Air, 2. Land, 4. Energy, 5. Noise b. Man-made environment: Items 4. Architecture, and 5. Historic Areas c. Public Facilities: Items 5. Refuse Disposal, and 6. Traffic Facilities d. Community Services: All Items e. Human Considerations: All Items 14. All construction within the development shall be conducted in accordance with the following conditions:" a. The developer shall make certain that no construction shall take place in areas which might cause disturbance to abutters prior to 7:00 a.m. on any work day, 8:00 a.m. on Saturdays, or at any time on Sundays, provided that nothing herein shall be deemed to restrict interior work or provided that it is exterior work which does not cause a disturbance. Work shall cease no later than 6:00 p.m. b. The developer shall take all actions necessary to minimize inconveniences to residents in the immediate neighborhood. Said inconveniences may relate to noise, vibration, dust, blocking of City roads or those concerns deemed appropriate by the appropriate Department heads of the City of Salem at the time of occurrence. The developer must advise abutters of work to be conducted near their premises at least twenty four hours prior to the commencement of said work. c. The Planning Board shall approve all agreements of the homeowners association regarding maintenance of the development, open space access, and all other matters pertaining to the operation and maintenance of the development, its land, utilities, infrastructure, and property. 16. The developer shall provide an access easement to the City of Salem which shall provide access to the footbridge siltation barrier for the purpose of inspection and/or maintenance when appropriate. 17. All building exteriors and designs shall be approved by the Planning Board prior to issuance of a building permit. 18. Prior_to the approval of the plan, the developer: a) shall submit and record a . duly executed restrictive covenant setting forth the conditions set forth herein in a form satisfactory to the Planning Board and b) shall revise the plan to include a reference to said covenant. Both such actions shall take place prior to the initiation of any construction. This restrictive covenant shall include any provisions the Planning Board deems appropriate to allow the City of Salem to enforce the maintenance obligations of the homeowners association as set forth herein. 19. The developer shall accept all conditions set forth herein and shall follow all requirements set forth in the Salem Massachusetts Subdivision Regulations and the Salem Massachusetts Zoning Ordinance, except as waived herein. 20.
