Dear Ms. Mattes:
On behalf of the property owner, Jamey Steadman, this letter requests that the Commission
grant a Certificate of Compliance for the subject property. Please find enclosed eight (8)
copies of the Massachusetts Wetland Protection Act Form 8A: Request for Certificate of
Compliance and an As-Built Site Plan (Sheet AB-1).
We have reviewed the subject Order of Conditions issued in May 30, 2018 under the MA WPA
and the City of Salem Wetlands Protection and Conservation Ordinance. An As-Built Site
Plan is enclosed showing work as of May 31, 2019. Based on the as-built plan and our recent
inspection, we certify that the work has been completed in substantial compliance with the
approved plan with the following deviations noted.
1) The building footprint was modified from that shown on the approved plan
resulting in an approximately 420 sf smaller footprint (2,100 sf approved; 1,680 sf
as-built).
2) The garage was changed from a “side load” to a “front load”. This resulted in a
smaller driveway (2,320 approved; 1,050 sf as-built). Site grading was modified
accordingly, as shown on the as-built plan.
3) The deck at the rear of the residence is slightly smaller than approved (240 sf
approved; 200 sf as-built).
4) As of this writing, the wetland boundary markers shown on the approved plan
have not been installed. Mr. Steadman plans to install these this week.
5) As of this writing, the front walkway remains under construction.
6) As of this writing, the site has not yet been fully stabilized. Hydroseeding is
scheduled for tomorrow morning.
Per Condition #43, roof runoff from the residence has been directed via gutters, downspouts
and roof leaders into a 1,000 gallon drywell as shown on the as-built plan.
