FIRE DEPARTMENT CERTIFICATE OF APPROVAL FOR BUILDING PERMIT
In compliance with the provision of Section 113.5 of the Massachusetts
State Building Code, and under guidelines agreed upon by the Salem Bldg.
Inspector and the Salem Fire Chief, the applicant for a building permit
shall obtain, the Certificate of Approval (see reverse side) and stamped
plan approval, -,from the Salem Fire Prevention Bureau.
Said application and approval is required before a building permit may be _J w
issued. The Massachusetts State Building Code requires compliance z w approval of the Salem Fire Department, with reference to provisions of u m3

Articles 4 and 12 of the Building Code, the Salem Fire Code, Massachusetts n: F p I

The applicant shall submit this application with three (3) sets of plans, W zo
drawn in sufficient clarity, to obtain stamped approval of the Salem Fire 2 H
Department. This applies for all new construction, substantial z w w
alterations, change of use and/or occupancy, and any other approvals a y a =
required by the Massachusetts General Laws, and the Salem. Fire Code. ¢ z � a
Exception: Plans will not be required for structural work when the
proposed work to be performed under the building permit will
not, in the opinion of the Building Inspector, require a
plan to show the nature and character of the work to be
performed.
Notice: Plans are normally required for fire suppression systems,
fire alarm systems, tank installations,, and Fire Code
requirements.
Under the provisions of Article 22 of the Massachusetts State Building
Code, certain proposed projects may not require submission of plans or
complete compliance with new construction requirements. In these
cases, provisions of Article 22, Appendix T, and Tables applicable,
shall apply. This section shall not, however, supersede the
provisions outlined in the Salem Fire Prevention Regulations, Chapter
148, MGL, or 527 Code of Massachusetts Regulations. All permits for
fire code use and/or occupancy shall apply for the entire structure;
fire alarm and/or smoke detector installation shall apply to the
entire structure based upon current requirements as per Laws and/or
Codes, but the existing structure may comply with regulations
applicable for existing structures.
Notice: Sub-contractors mal also be required to file individual
applications for a hire Department Certificate of Approval
for the area of their work. Such sub-contractors shall file
an Application to Install with the Fire prevention Bureau
prior to commencing any work for those areas applicable.

Sirs:
As a result of an inspection for the fire alarm system located at
530 Loring Ave. Salem, Ma. , which you have installed without prior
permit to install and without first obtaining stamped approval of
plans, the following items shall .be noted: 1.
l. -As of this date, the system is considered illegal.
1. Occupancy of the building shall cease immediately and court action
shall be initiated against all occupants on April 10, 1984, if
said occupancy shall continue without a Certificate of Occupancy.
3 . The following items shall be required of your company to obtain
approval of the system installed at 530 Loring Ave. Salem, Ma.
a. You shall make an appointment with myself at my office to
determine such action to be taken against you with the Board
of Plumbers and Gas Fitters, for failure to comply with the
provisions of the Salem Fire Code and the Salem Building Code.
b. The one set of plans submitted is insufficient for stamped
approval of this office. Three sets of plans in sufficient
clarity and detail are required for review of this office.
c. Upon inspection it has been found that you have installed
the standpipe at the rear stairway, instead of at the front
stairway. The access to the standpipeis required at the front
entry, where its use will be easily accesable to the fire
supression forces.
d. You have installed the system without the required gauges at
the O.S.&Y. valve , located in the basement.
e. A detailed drawing of the O.S.& Y. valve shall be required,
showing all details of the valve, the fire department connection,
gauges and check valves.
f. An inspectors test gauge shall be required at the top most portion
of the stanpipe system.

g. An appointment shall be made for a test of the system by the
Salem Fire Department. Before the appointment you shall provide

that a two hundred pound test has been conducted by you, on
the system, as required by N.F.P.A. Standard #13 and #14. Said
Certificate shall be on a form approved by the Insurance
Services Office, for rating purposes.
You are hereby ordered to comply with the above requirements for
installation of a standpipe and sprinkler system, on or before
April 10, 1984, or such furthur action as the law allows shall be
initiated.

Sir:
As a result of an inspection made on March 30, 1984, the following items
shall be noted which affect the signing of the Certificate of Occupancy
for your new office condominium structure.
1 . The fire alarm test was conducted and the system proved satisfactory,
except for the need to provide an audible trouble buzzer gn the fire
alarm panel . The electrician shall' provide same and request a reinspection.
2. The sprinkler and standpipe system are not approved as of this date, and
a..copy of the order to H.W. Larson & Son, Inc. is enclosed.
3. The occupancy of this building shall be considered illegal , until all
tests are conducted and the Certificate of Occupancy has been issued.
4. You shall provide at least one all purpose fire extinguisher with a
Classification of 2A403C, properly- hung on a bracket, in the front entry
stairway area, on each floor, and one extinguisher in the rear stairway
at the lower level , to cover the basement area.
You are requested to provide the necessary items for occupancy, or such action
shall be taken as the law provides.

Dear Dick:
I have a client who is going to purchase Lot A and Lot B, which
are combined as one lot. The total lot area then would be 26, 877 square feet,
more or less.
My client wishes to put up a 23-unit motel which will comply
with the density, set back, sideline, rear yard and front yard requirements.
Before I advise him to go forward, I wonder if you could look at this plan and
see how far back the zoning line goes, and approximately where the B-2 area
ends and the R-3 begins.
If any other information is necessary, Lennie, my associate,
will get it for you, but if you could note the zoning line on this plan, I would then
be able to advise my client by tomorrow morning when he comes in.
