Enclosed you will find an Application for Child Care .Exemption and an EEC policy statement for the Criteria for Exemption from Licensing. Please use this policy as a guide to complete your exemption application. Please return your exemption -request -by _ 2/22/08 to my attention. I will- review -your exemption request and- noEify you whether or not you meet the definition of an exempt program. As I shared with you during my visit, as long as you receive either a license from EEC or an exemption letter from EEC you will sti-11 be eligible for vouchers . . During my unannounced health and safety visit, I reviewed with you my concerns regarding the condition of your building. I have been in touch with the building and health department to coordinate our efforts in assisting you in addressing these concerns . EEC views many of these concerns as serious and in need of immediate attention. - The roof is leaking in several locations through out the building. There were no, buckets to catch this water. The water dripped onto surfaces freely. - There appears to be mold growing in the pool area on the shelves, hose, wall etc . Items are not being stored in a sanitary manner in the pool area. - The floor is rotting in several areas through out the program. - -" All the vents in the program have evidence of dirt and `possible mold. - The pool inspection expired on 12/2007. You were mixing infants, toddlers, and preschoolers in one - group. ----_--; You were using the previously licensed preschool room for infant, toddler,' and preschooler art projects when it was not set up and safe for child care use. There was a sewing,o` machine, cleaning equipment, and other 'non child care items--7----- being stored in this room. The sinks in this room were dirty and the backsplash was rotting. - The equipment through out the program is decomposing. The surface of many tables and chairs was worn away making it impossible to dis €ect these surfaces ade, a %;.y. - Two toddlers were observed sitting in a table seat for more than 30 minutes while the remaining 4 toddlers were allowed . to play and walk around. - The program has several turtles and children were observed picking up the turtles, handling them and not washing their hands . The Department of Public Health states that infants, children, the elderly and people with weakened immune systems should be prevented from touching reptiles (lizards, snakes, and turtles) and their cages since there is a risk on contracting salmonellosis . Munroe Inspector of Buildings One Salem Green Salem, MA 01970 Dear Mr. Munroe: This is in response to your;letter:of_December=3_,:1985 regarding the dumpster at the Sol`arterre-e-em located at 145 Loring Avenue. The whole issue dealing with the rodent activity at the Loring Avenue and Monroe Road area began in May 1985. At that time illegal dumping was taking place and an extensive accumulation of overgrowth and debris was present. This. department requested then that all the abutting property owners take effective measures to correct these problems (see enclosed letter) . The GTE Corp. (Sylvania) enacted an extensive program with included the removal of all overgrowth and debris, the erection of a chainlink fence along Monroe Road and the Loring Avenue property to prevent further illegal dumping, and landscaping to beautify the area. Mr. Bruce Dellachiaie of 139 Loring Avenue also removed overgrowth and debris from his property and conducted landscaping. The Solarterre-e-em at 145 Loring Avenue has been inspected by this department on numerous occasions. This facility is licensed as a day care center and a recreational camp and meets all the requirements of this department as well .as the state Office For Children. In addition, the Solarterre-e-em holds a valid permit for its wading pool. Presently the Solarterre-e-em is under contract with B&B Exterminators of Lynn to provide monthly service for interior and exterior control of rodents. William Monroe Building Inspector 1 Salem Green Salem, MA 01970 Dear Mr. Monroe , Persuant to our recent conversation, I would like to register a complaint concerning the location of the trash dumpster being used by the Solarterre-e-em Day .Care Center on 145 Loring Avenue. This complaint is in respect to the city ' s back and side setback re- quirements, I understand that said dumpster is considered a permanent structure since it is not moved on a daily basis. It is currently situated approximately 1 foot from my 141 Loring Avenue property line and approximately 3 feet from Sylvania' s land in the rear . In addition to being a constant source of aggravation (with respect to 6a.m. and 7 a.m. pick-ups) , it is also causing a considerable health and sanitary concern as rodent activity in the vicinity is prevalent 24 hours a day . The Salem Board of Health has received numerous complaints over the past 6 months from ourselves and adjacent neighbors . Their response has been that there was no recourse from a health standpoint and even sdggested that we should incur the expense and contract an extermin- ator. We received a copy of a letter from R. Blenkhorn to the Cen- ter ' s owner , Patricia Welch , dated October 22 , 1985 , in which a request was made ,that the area be kept clean and the dumpster be kept closed with a locking mechanism at all times. To date , nothing has been done to meet this request. Our lack of satisfaction has led us to this as- w1lat we believe is our only current course of action and we would appreciate your prompt attention on this matter . .
