As a result of an inspection this date of the premises, structure ,
open land area or vehicle owned, occupied or otherwise under your
control , the following recommendations are submitted and shall serve
as a notice of violation of fire 'laws . These recommendations are
made in the interest of fire prevention and to correct. conditions
that are or may become dangerous as a fire hazard or are in violation
of law.
You are hereby notified to remedy said violations named below within
seven days , of above date.
Such further action will be taken as the law requires , for failure
to comply with the above requirements within the stipulated time .
(Reference : General Laws of Commonwealth of Massachusetts, Chapter 148
Section 30 ; and the Salem Fire Code Article 1 .)
1. Found illegal gas space heater unit, with front glass removed ,
and piped before meter, with open flames, and adjacent to combustibl ,
materials, (55 gal. drum of range oil and chair) t and apparently
piped into a combined flue with a domestic hot water heater and the
oil burner unites Boston Gas Co. & Salem Gas Inspector were notified
and unit was disconnected as a `fi.re hazard. Drum to be removed.
2. A U. S. Carlin oil burner has been installed in an improper manner,
without a permit for. same. The unit is resting on a copper pipe
stand, which was in turn on a piece of sheet rock laid on the floor.
This stand could be easily knocked over and the burner would have
no support. The burner is installed into the regular fire door,
and no means is provided for viewing the fire chamber and rate of
fireing of the nozzle. Piping for oil line is exposed to probable
damage at front of burner. Electric wire are exposed from Service
mans control box at front of burner. The burner was, pulled from
the control box to stretch the wires, so the burner could be placed
into the new location in the boiler door. The emergency switch
was not changed to outside the area where the boiler is located.
The low water-cutoff controls are rusting away and should be replace
Fuel oil has been delivered to the premises without a posted permit
for the storage of fuel oil to the Carlin burner.
3. Several open electrical boxes and electrical wire problems were
found.and- the City electrician_ was notified.
