Dear Mr. Dibble:
Attached please find 2 copies of a Proposed Construction Plan and a Request for a
Determination Form for the above referenced lot. The plan depicts the existing single
family dwelling and associated surrounding features. The applicant requests permission
from the Commission to replace the existing wood and stone foundation with a concrete
foundation as described in his letter dated June 10, 1996. The plan shows the limits of the
the haybales which are proposed for erosion control. We respectfully request that the
Salem Conservation Commission schedule the project for discussion at the next available
hearing.

The Salem Conservation Commission will hear a Request for Determination of Applicability under
the Wetlands Protection Act, Massachusetts General Laws, Chapter 131, Section 40, at the request
of the David Perkins, 30 Winter Island Road, Salem, MA 01970. The proposed work located at
30 Winter Island Road, consists of raising the building to install a typical concrete foundation and
to complete appropriate sill work to restore the structural soundness of the dwelling. The request
will be heard on Thursday, July 11, 1996 at 7:00 p.m. in the second floor conference room at One
Salem Green.

This letter is in regard to my primary residence at 30 Winter Island Road in Salem,
Massachusetts and the reasons I am seeking construction approval. The existing building was
built in the early 1920's and at that time the foundation was constructed of stone and wood.
Today the building is in need of a new foundation and sill repair/replacement. I am requesting
approval to raise the building to install a typical concrete foundation and to complete appropriate
sill work to restore the structural soundness of the dwelling.
