General Laws, 105 CMR 400.000: State Sanitary Code, Chapter 1: General
Administrative Procedures and 105 CMR 410.000: State Sanitary Code, Chapter
II: Minimum Standards of Fitness for Human Habitation, an inspection ,was

occupied by John Fernandez/Common Areas— This inspection was
cmnducted by V. Moustakis/Tenant Salem Health Department, on
2/20/86 at 11 :30 A.M.
Based upon said inspection, you are hereby ordered to take the following
action within 24 hours of receipt of this order:
Kitchen ceiling is leaking according to tenant - This leak must be
investigated and repaired.
Must investigate and repair kitchen radiator valve - Tenant states it
spurts out water.
Must investigate and repair bathroom radiator this is leaking according
to tenant.
Tenant states bathroom sink drains slowly - Must check out and correct.
Replace smoke detector battery.
Must repair living room ceiling - evidence of a leak.
Must provide back door 1st floor leading to exterior with a proper locking

and must be affixed.

beverages; illegal keeping, sale, or manufacturing of controlled
substances .
You are hereby given due notice of this condition and you are
hereby ordered to take all reasonable measures necessary to eject
from said premises, the person occupying the same, as soon as it
can be lawfully done.
You should be further advised that your failure to abide by
• this notice .will be considered a crime punishable by a fine of not
