Attached is a revised notification. The revisions involve two changes to our original submittal. First, the original demolition contractor was unable to proceed with work as planned. Therefore, we had to change the demolition company. Second, the start date had to be modified to accommodate the change in demolition companies. The revised notification reflects the new names, addresses, and telephone numbers of the project personnel. All other information on the form remains unchanged. Currently, we are planning to mobilize the new demolition contractor on Thursday, June 27. We anticipate the work to be completed in approximately five to six weeks. SPILL CONTROL & CONTINGENCY PLAN OSRAM SYLVANIA INC. 60 Boston Street - Demolition Project Salem, MA 01970 (Order of Conditions #64-243) May 20, 1996 The following spill control and contingency procedures will be adhered to throughout the building demolition operations and associated activities at the above-referenced project: 1. Refueling operations will not be permitted within 100 feet of any wetland resource area. 2. Only fueling of equipment using Type 2 diesel fuel will be permitted on site. 3. Stationary, on-site storage of fuels will not be permitted. 4. Tank trucks containing fuel for storage will not be permitted to remain on site unattended, overnight, or during off-hours. Tank trucks will be permitted to remain on site only during refueling operations. 5. All refueling operations will be by licensed tank trucks and associated personnel who are licensed for refueling operations. 6. On-site refueling operations will only take place in designated areas. 7. Designated refueling areas will be established so as to minimize potential infiltration of contaminants into any wetland resource area. Refueling will be restricted to paved areas to provide additional protection of soils and groundwater. 8. Designated refueling areas will be clearly established and identified to on-site personnel engaged in refueling operations. Proper refueling procedures, including this Spill Control & Contingency Plan and Order of Conditions #64-243, will be provided to on- site personnel engaged in refueling operations. 9. All on-site refueling operations at designated refueling areas will include spill control materials and equipment that are readily accessible during refueling. Spill control materials shall include, but not be limited to: (a) petroleum absorbent pads, pillows, and/or booms designed specifically for recovering oils and petroleum-based liquids caused by incidental spillage; (b) petroleum absorbent particulate used to control and absorb petroleum-based liquids caused by incidental spillage; (c) protective safety glasses and gloves for personnel exposed to petroleum products; (d) hazardous waste bags with ties and labels used for containing hazardous materials; and (e) a containment drum for temporary storage and proper disposal from the site of hazardous materials. 10. Incidental spillage is understood to mean the inadvertent discharge of small amounts of fuel that may occur during normal refueling or demolition operations. In the event of incidental spillage, on-site personnel shall utilize spill control materials to contain and reclaim discharged liquids. Contaminated materials shall be placed in a designated drum for temporary storage and legal disposal according to federal, state and local regulations. 11. Erosion control procedures using haybales, and siltation fencing and screens will be maintained throughout demolition operations as required in the Order of Conditions. In the event of a significant spillage of fuel resulting from unexpected or catastrophic conditions, the situation would be defined as an "emergency." In the event of an emergency situation, on-site personnel will do the following: a. Halt on-site construction and/or demolition activities and relocate personnel away from the emergency. b. Notify local enforcement and fire prevention officials of the emergency. c. Contact Clean Harbors Environmental Services, Inc. of Braintree, Massachusetts to implement emergency containment and control procedures. d. Contact GZA GeoEnvironmental, Inc. of Manchester, NH to provide environmental oversight of emergency procedures. e. Notify the City of Salem Conservation Commission and the Massachusetts Department of Environmental Protection of the emergency. f. Discontinue on-site demolition activities until emergency procedures are completed and regulatory authorities approve recommencement of work. Site storm drainage system is connected to the City of Salem drainage system, which in turn, drains into North River Canal. Environmental protection barriers, including siltscreen covers for catch basins and area drains, are designed to protect the resource area. ATTACHMENT 2 PART IV: Mitigating Measures The whole site slopes toward Bridge Street. Storm water is collected by catch basins with underground drain pipes connected to the City of Salem drainage system. The measures taken to protect the nearby waterway are: 1. Filter barrier will be placed on the property line along Boston Street, Bridge Street and the eastern line of the property to the existing concrete retaining wall. The filter barrier consists of a combination of siltscreen placed directly on the fence or haybales. 2 . Catch basins and area drains will be covered with siltscreen cover secured to the ground. The screen shall be maintained and cleaned as required to provide the required drainage. 3. Piling debris in the flood plain is not allowed and shall be moved away the same day and prior to anticipated storms. 4. All work shall conform to the above-referenced Notice of Intent (NO[), site plan and supporting documents and those final specifications which shall be filed with the Commission prior to construction. Any change made or intended to be made to the approved plans shall require the applicant to inquire of the Commission in writing whether the change is substantial enough to require the filing of a new Notice Of Intent (NOII. 2. Members and agents of the Commission shall have the right to enter and inspect the premises at all reasonable times to evaluate compliance with the conditions in this Order. The Commission may require the applicant to submit additional data or information necessary for the Commission to conduct that evaluation. 3. Upon completion of the project, the applicant shall submit, with the request for a Certification of Compliance, an affidavit, prepared by a professional engineer or land s surveyor registered in the Commonwealth of Massachusetts, stating that the site has been developed, in accordance with the requirements of this Order of Conditions, the referenced site plan and supporting documents. 4. This Order shall apply to every successor in control or successor in interest of the property described in the Notice of Intent and accompanying plans or part thereof. In the event this land or any part thereof changes ownership before or during construction, the current owner shall notify the new owner, by registered mail, of this Order prior to the transfer of ownership and shall forward proof of this notification to the Commission. 5. Prior to commencement of construction activity, a haybale barrier shall be placed downgradient of construction to prevent siltation to the adjacent wetland. Haybales shall be anchored with stakes and entrenched 4 to 6 inches in depth. Trench material shall be deposited on the upgradient side of the haybale barrier. Haybales shall be inspected daily; those showing signs of deterioration shall be replaced immediately. The haybale barrier shall remain in proper functioning condition until all disturbed areas have been stabilized. A siltation fence, as per plan submitted to Conservation Commission, shall be installed. 6. Additional erosion and sedimentation control measures deemed necessary by the Commission shall be instituted by the applicant. Any siltation caused onto any wetland or waterway area shall be promptly reported to the Commission. Such areas shall be restored to the satisfaction of the Commission. Screening shall be placed over catch basins as per plan submitted to the Conservation Commission. Screens shall be inspected and cleaned on a daily basis. 7. During construction, no debris, fill, or excavated material shall be stockpiled within 100 feet of designated wetlands. 8. Equipment servicing and overnight equipment storage must be conducted outside the buffer zone. Equipment must be maintained to prevent leakage or discharge of pollutants. 9. No spill or discharge of petroleum products or other pollutants will occur within 100 feet of any wetland resource area. Owner shall submit to the Conservation Commission a contingency plan to control in advertent spills. The plan must be reviewed and approved by the Conservation Commission before commencement of Building Demolition. 10. * Use or storage of rock salt (sodium chloride) or other toxic de-icing chemicals is prohibited within 100 feet of the resource area. Any arrangement for snow removal shall stipulate the ban. 11 . Notice shall be given to this Commission no more than 2 weeks nor less than 2 days prior to the commencement of construction activities.
