Evans Beverly Savings Bank 175 Cabot Street RE: 38 Leach St. Beverly, MA 01915 lst floor Dear Mr. Evans: On December 14, 1979 I made an inspection at the referenced premises as requested by Laurie S. Denno, Program Director of the North Shore Assoc. for Retarded Children. The purpose of this inspection was for group residence certification. I found that the premises was already occupied by four "clients" and a manager-supervisor John Lyness. This occupancy without a certificate of occupancy is a violation of the Building Code. I further find that I would not be able, at this time, to certify the premises as a "Group Residence under Massachusetts State Building Code Section 424.0. Enclosed is a copy of this section with the violations illuminated in yellow. You are hereby advised that you shall commence work within seven (7) days of receipt of this notice and that said work shall be completed within thirty (30) days.
