egress stairs and handrails are in a dangerous state of disrepair in violation of the
State Building Code, Section 115 of 780 CMR.
The stairs and handrails needs to be repaired or replaced immediately. You are
hereby directed to enlist the assistance or a Registered Architect or Professional
Engineer or a CSL Licensed contractor to evaluate the existing structure and
indicate necessary repairs or to design a code compliant stair.
You are also hereby directed to address this violation within 5 days of this notice.
Failure to do so may result in further actions being brought against you,up to and
including fines and the filing of criminal complaints at district Court. You have the
right to appeal this decision to the Bureau of Building Regulations and Standards,
One Ashburton place, Boston,Massachusetts.

Dear Owners,
This Department sent a violation notice over 10 months ago. To date, no action has been taken to
address the code violation. As noted in the previous violation notice, the rear egress stair is in disrepair
and is dangerous enough that it is noted at Salem Fire Headquarters for the.Firefghters to stay off.
This failure to Maintain a Required means of egress violates section 116,1 through 116.5 of the eighth
edition of the Mass Building Code with Mass amendments. This stair needs to be replaced or repaired
immediately. The repair/replacement needs to be permitted and constructed by a licensed individual .
Failure to respond and begin repairs will result in daily 21-D tickets and further enforceruent
activities.
If you feel you are aggrieved by this order, you have the right to appeal this order to the Board of
Buildings ,Regulations and. Standards.(BBRS)

At a recent inspection of the above property I observed that the exterior egress stair at the
above address is in a dangerous state of disrepair in violation of the State Building Code
Section 115 of 780 CMR.
This stair needs to be repaired or replaced immediately.
You are hereby directed to enlist the assistance of a Registered Architect or Professional
Engineer to evaluate the existing structure and indicate necessary repairs or to design a
code compliant replacement stair.
You are hereby directed to address this violation within 21 days of this notice.
Failure to do so may result in further actions being brought against you, up to and
including, fines and the filing of criminal complaints at District Court.

On the above date I inspected the above listed three family dwelling for the
compliance of automatic smoke detedtors. During the inspection I noted that
all the doors both front and rear, on all floors , which are between the stair-
wells and the units , are hollow core type doors.
This property is in the process of being sold, the present owner is Mr. David
Walsh, 8 Fox Run Road, Danvers, MA 01923. The realtor is Mr. Jack Hyland,
115 Lowell Street, Suite #1, Peabody, MA 01960. Mr. Hyland was notified of
these violations in order that he may notify the buyer of the conditions.
I also informed Mr. Hyland that the present owner must contact your office for
the purpose of installing the proper doors according to the Massachusetts State
Building Code.

This office has received a complaint from the City of Salem Fire Prevention
Office on possible building code violations at the above referenced property.
Upon inspection on April 3, 1990, I found you to be in violation of the
Massachusetts State Building Code, Section 113 (permits) . There is no record at
this office of any building permits being issued for the property.
Violation Penalties:
Anyone who shall violate a provision of this code shall be puni-
shable by a fine of not more than one thousand dollars ($1,000)
or by imprisonment for not more than one year, or both, for each
violation. Each day during which any portion of a violation con-
tinues shall constitute a separate offense.

R,E Building Code Violation
Dear Owners,
This Department sent a violation notice over 10 months ago. To date, no action has been taken to
address the code violation. As noted in the previous violation notice, the rear egress stair is in disrepair
and is dangerous enough that it is noted at Salem Fire Headquarters for the Firefighters to stay off.
This failure to maintain a required means of egress violates section 116,1 through 116.5 of the eighth
edition of the Mass Building Code with Mass amendments. This stair needs to be replaced or repaired
immediately. The repair/replacement needs to be permitted and constructed by a licensed individual .
Failure to respond and begin repairs will result in daily 21- D tickets and further enforcement
activities.
If you feel you are aggrieved by this order, you have the right to appeal this order to the Board of
Buildings ,Regulations and Standards.(BBRS)

above address is in a dangerous state of disrepair in violation of the State Building Code
Section 115 of 780 CMR.
This stair needs to be repaired or replaced immediately.
You are hereby directed to enlist the assistance of a Registered Architect or Professional
Engineer to evaluate the existing structure and indicate necessary repairs or to design a
code compliant replacement stair.
You are hereby directed to address this violation within 21 days of this notice.
Failure to do so may result in further actions being brought against you, up to and
including, fines and the filing of criminal complaints at District Court.
You have the right to appeal this decision to the Bureau of Building Regulations and
Standards (BBRS)
