Unit No. 1, 2, 3, and 4. New England Power Company, Salem, Massachusetts". This report outlines the results of compliance testing conducted on July 21,22, and 23, 1986 on Salem Harbor Units 1, 2, 3, and 4. Unit No. 4 was tested for particulate emissions while units 1, 2, 3, and 4 were tested for nitrogen oxides emissions. The nitrogen oxide testing was for informational purposes only. A summary of the reported particulate emission rates follows: 1. Unit No. 4 had an average particulate emission rate of 0.058 pounds per million B.t.u, during normal operation and an average particulate emission rate of 0.072 pounds per million B.t.u. during soot blowing. A comparison of these results with the regulatory requirement indicates that Unit No. 4 is in conformance with the particulate emission rate of 0.12 pounds per million B.t.u. and the Department hereby grants FINAL APPROVAL of the particulate portion of the compliance test. The submitted information bears the seal and signature of Mr. John W. Lebourveau, Massachusetts P.E. No. 9344. This review of the submitted information by Department engineers indicates that New England Power Company proposes to burn on-site and off-site generated waste oil in Unit No. 4 at Salem Harbor Station. The on-site generated waste oils will consist of specification and non -specification used lube oil from steam turbines, compressors, and mobile equipment, such as tractors, which are used on-site. Off-site generated waste oil consists of specification and non -specification waste oils, which are generated at and transferred from diesel electric generating stations at 26 Vincent Street, Gloucester, and 78 Water Street, Newburyport, Massachusetts. These facilities are owned by Massachusetts Electric Company and are operated by New England Power Company. The on-site generated waste oils are stored in 55 gallon drums then transferred to a dedicated 1000 gallon underground storage tank. Off-site oil is transported in 55 gallon drums and added to the same tank. This oil is pumped from this 1000 gallon tank into the facility's fuel oil return line which returns fuel oil to the fuel oil storage tank being used as a day tank. This will provide adeqbate mixing of the oils. These oils will be burned exclusively in Unit No. 4, a Riley Stoker Corporation boiler having a maximum firing rate of 33,000 gallons of No. 6 fuel oil per hour. There is also a waste oil tank in the oil supply pump house which collects drains from pumping equipment and discharges to the same return line. It is projected that approximately 12,000 gallons per year of waste oil will be collected and burned in Unit 4. Approximately 130 to 1.70 million gallons of No, 6 fuel oil having a sulfur content not in excess of 2.2 percent by weight is currently burned in Unit 4 per year. That should any nuisance condition(s) be generated by the operation of this facility, then appropriate steps will immediately be taken by New England Power Company to abate the nuisance condition(s). 2. That the ability of the equipment to maintain an emission rate at or below 0.12 pounds of particulate per million B.t.u. shall be demonstrated when and if, in the opinion of the Department, such should prove necessary. 3. That no solvents, other waste oils, or any other liquid wastes shall be added to the waste oil and burned or burned separately in the subject fossil fuel utilization facility without first having received written Department approval. 4. That off -specification oil not generated on-site shall be transported via a licensed hazardous waste transporter and in accordance with all applicable Hazardous Waste Regulations. All waste oils not generated on site shall be presumed to be off -specification unless an independent analysis is conducted on each shipment of waste -oils. Analysis shall include as a minimum, testing for the following constituents; sulfur, total halogens, lead, arsenic, cadium, chromium, and PCB content. 5. That New England Power shall apply for and obtain a recycling permit through the Department's Division of Hazardous Waste. 6. That New England Power shall provide quarterly reports to the Department which indicate quantity of on-site and off-site waste oil cumbusted at the described facility during the most recent three month period. Also, any laboratory analysis performed shall be due on January 15, April 15, July 15, and October 15 of each year. The first quarterly report shall be submitted on or before October 15, 1987 and shall cover the months of July, August and September 1987. , New England Power Company Page -3- Failure to comply with any of the above stated provisions will constitute a violation of the "Regulations," and can result in the revocation of the approval granted herein to operate the described facility. The Department may also revoke this approval if the construction work is not begun within two years from the date of issuance of this approval, or if the construction work is suspended for one year. For Air Quality Control purposes, an Environmental Notification Form is not required to be submitted for this project since it is exempt under the Environmental Protection Regulations of the Executive Office of Environmental Affairs, and the project has been determined to•cause no significant damage to the environment. CPl'Y UN SALEM HEALTH DEPT New England Power Company RE: SALEM - ERB-N86-1275 Salem Harbor Station Oil/Hazardous Material 24 Fort Avenue Release/Threat of Release at Salem, MA 01970 WRITTEN CONFIRMATION OF NOTICE OF RESPONSIBILITY/SUMMARY OF RESPONSE ACTIONS TAKEN/REQUEST FOR INFORMATION Attention: Jack Pitman, Environmental Engineer Dear Sir: As a result of an investigation conducted by the Department of Environmental Quality Engineering (DEQE) on December 16, 1986, the Department has determined that there has been a release/continued threat of release of #2 fuel oil in the area of fuel oil storage tank D-5 adjacent to the discharge channel. Such incident is governed by Chapter 21E of the General Laws of Massachusetts (herinafter "M.G.L. Chapter 21E"), the Massachusetts Oil and Hazardous Material Release Prevention and Response Act, which was enacted on March 24, 1983. The Department has reason to believe based on available information that you are, under the provisions of M.G.L. Chapter 21E, Section 5(a), responsible for the occurrence of such release/threat of release. On December 16, 1986, Department personnel verbally notified Robert DeHart of your company of your responsibility for such release/threat of release and gave him one copy of a document prepared by the Department and entitled "Brief Synopsis of M.G.L. Chapter 21E, the Massachusetts 011 and Hazardous Material Release Prevention and Response Act." He accepted such responsibility at that time. Your company's acceptance of responsibility for such release means that your company has entered into a contract with Geotechnical Engineers of Winchester, MA to take all necessary remedial actions (i.e. assessment, containment and/or removal actions) relative to such release. On July 20, 1987, the Department received the hydrogeological report prepared by your consultant. The Department is in general agreement with the findings and - conclusions of the consultant's report, however, one problem that still exists is the identificaiton of the source of the oil release. Should the discharge into the waterway recur or should new information concerning the source of the oil be revealed, the Department must be notified immediately and your company will be required to undertake further investigation/remediation. Until such time, your company should institute a periodic monitoring/sampling/analysis program of groundwater via the existing observation well@ to determine changes, if any, in the condition of the groundwater. Reports of your findings should be submitted to the Department's Site Assessment Section for review and refer to case #N86-1275.
