The first and second floor porches are unsafe. 2. Water leaking through lights in hall. 3. Smoke detectors don't work in hall. 4. Windowsill rotted out. 5. Water leaks in bedroom I" floor. 6. 2"d floor den—water leaks. 7. Gas grill can't stay on 2"d floor porch. 8. Basement dryer needs to be vented. 9. Toilet in basement needs to be removed. 10. Third floor apartment is illegal. The 3`d floor apartment must be vacated within thirty (30) days. If you feel aggrieved by the building issues your appeal would be directed to the BBRS, State Building Code Appeals Board. Failure to respond to these orders within fifteen (15) days will result in legal action being taken against you in Salem District Court. This work was performed without benefit of an opening permit or an inspection of the work, all in direct violation of City Ordinances. In addition, the sidewalk, after seven days, has not-been repaired. This repair must be made in accordance with D.P.W. regulations. I have advised you personally that you have previously violated City Ordinances regarding opening permits and I herewith notify you that this present violation is the last,one that will be tolerated.
