need approval from the City of Salem Board of Appeals .
Only one kitchen is allowed in a dwelling unit . A
separation wall dividing a condo unit is not allowed.

As the result of an inspection of the premesis, structure, open land area,
or vehicle owned, occupied, or otherwise under your control , the fol-
lowing recommendations are made and shall serve as a notice of a violation
of the laws , ordinances , or regulations pertaining to the prevention of
fire and the protection of life and property.
As a result of the inspection and the incident of fire in your condo the following
is hereby ordered:
1- The immediate removal and disposal of all rubbish in the kitchen area. Notifica-
tion to this office when the kitchen is clean so that it can be reinspected.
2- The immediate attention to the cleaning of all mons. The condition in this unit
is &hdirect violation of the Massachusetts General Laws, Chapter 148, Section 5.
It also jeopardizes the safety of all the occupants in this dwelling.
3- Systematic procedure of cleaning will be under the supervision of this office.
This office will inspect the unit on a weekly basis until such time it beooi[es
safe to you and the other tenants.
4- The floor in the bathroom near the tub is punky. This condition will be investi-
gated by the building inspector.
