Due to a complaint received by the tenant in
apartment #4 on June 22, 1998 , I conducted an inspection
of your property located at 303 Lafayette Street and found
the following violations;
I . Illegal apartment located in the basement .
2 . Cracked waste lines in basement. v�
3 . Storage of rubbish in egress . -
4 . Sink and toilet not draining properly in apartment #4 .
Please notify this department within fifteen ( 15 )
days upon receipt of this letter, to inform us as to what
course of action you will take to rectify these
violations . Failure to do so will result ,in legal action
being taken against you in Salem District Court .

Dear Mr. Pelletier;
On March 11, 2005 1 was called to the property located at 303 Lafayette Street by our Senior
Sanitarian,Janet Dionne,to inspect damage to the ceilings in one of the top floor apartments. It is obvious
that there has been a leak problem going on for some time. Ceilings in this unit as well as both the front
and the side hallway, have fallen in, and there is evidence of previous repair to these same areas. As yet
there is no evidence of further structural damage,but the continued leaking of the roof in these areas is a
cause for concern. During the repairs to these ceilings an inspector from this office must be allowed to
view the exposed structural elements so as to assess the extent of the damage being caused by this
continued leaking.
Under the provisions of 780 CMR section 103, it is the responsibility of the property owner to
maintain his property in a safe and sanitary condition. You are therefore ordered to begin immediately to
make the needed repairs to both the roof and the effected ceilings within. This is a commercial property,
therefore the necessary permits must be secured by a licensed contractor, and all necessary inspections
must be conducted. Please inform your contractor that this office must be contacted so that an inspector
can verify the structural integrity within the effected areas. Repairs to this damage must commence within
one week from the receipt of this letter.
