Baumgartner Executive Director United Cerebral Palsy Association of the North Shore, Inc. 103 Johnson Street Lynn, Massachusetts 01902 REPORT - re: Renovation Project 275 Lafayette Street Salem, Massachusetts During our meeting with the Salem Building Inspector on site today, the following items at the referenced project were found to be incomplete, defective, or to require additional action: • After testing a number of exit light in broad daylight with artificial light in the room, TMR offered the inspector a submission of exit light specifications and description (cuts) next week. • Rails at exit door (existing at left and right sides of building): install balusters and sub-rail system so that children cannot fall off side of landing. • Adjust door pull resistance: make lighter (not to exceed 15 pounds of pull). • Typical throughout building: ID electric panels and circuits. • Install locking circuit breaker on fire alarm circuit. • Install receptacle cover in office at right rear. • Typical throughout building: room designation signs. • At first, the inspector required cuts for observation room windows but the etched tempered stamp was identified: no work required unless TMR decides to send cuts. • Install illuminated exit signs in large classroom and small classroom adjacent (right side). • Inspect and repair latches on side exits: repair as required for proper operation and security. • Install grommet(s) at telephone boxes where wires penetrate wall. • Install end caps on all baseboard radiators. • Replace dented smoke detectors. United Cerebral Palsy Association of the North Shore, Inc. Page 2 Inspection Rpeort • Push paddles at former main entrance doors (Summit Avenue) is required: also glass in same doors need repair. Inspector requires that both doors operate and have push paddles. • Install basement access door with lock. • Install closer on storage room door near back entrance. • New emergency lighting in basement w/illuminated exit signs required at two exits. • Low voltage wire pair at basement ceiling is exposed and incomplete`. remove or install with conduit. • Romex-style wire is exposed at basement ceiling (hot water circuit). This wire should be replaced in conduit - if not routed behind finish. • Ash pit at boiler clean and empty: dispose of debris. • Current boiler service tags indicating proper performance are required. • Plate screws on various covers are necessary. • Boiler room door must be self-locking door w/closer and must shut completely by itself. A lock should be operable without key from inside the room such that occupant may readily exit during an emergency. • Same as rooms at first floor: boiler room designation. • Same as rooms at first floor: label panel in basement. • Old gas service pipe which enters from Summit Avenue through basement wall must be capped at inside face. Furthermore the gas company must give notice that same service is shut off at the street (in writing). • Repair glass at basement window sash - Summit Avenue end. • Interior basement stairs must have one hand rail (wall mounted) at one side minimum. This work shall be performed on or before March 31, 1998: the date for final state and local inspections before occupancy. If you require additional information, please advise.
