Dear Mr. Daly:
On May 16, 1977 I inspected the above referenced property known
as the Fairfield Tourist Home. The property, at that time, was owned
by Jon-Heath Realty, 63 Liberty Street, Middleton, Massachusetts.
The inspection revealed there were two (2) units on the first
floor, five (5) units on the second floor and four (4) units on the
third floor for a total of eleven (11 ) rental units.
This office has no record of any Board of Appeal approval to
increase the number of units. Therefore, the number of units stands
at eleven ( 11 ) .

1 . The purpose of the meeting was to review the interior stairwell and
to make decisions regarding the closure of the stairwell space . At
a previous meeting Mr. Louis Salemi suggested a joint meeting of the
Building Inspectors , the Architects and Real Estate Investments . The
decision was made by all that the doors will have door closures as
requested by the Building Inspectors and that the doors that face the
interior of the stairs will be changed to either a solid core door or
a six-panel metal door . The owner can select the type preferred as long
as there is a one-hour rating. The door closures will provide security
of spaces at each end of the cooridor at the other exits . Emergency
lighting will be provided and lighting shall be maintained at all
times at exit-ways on a 24-hour h^cis
2 . It was agreed by all that the stairwell is within the intent of
Article 22 , Appendix T of the State Code even though not all aspects
have been covered. Mr . Salemi also feels that what we are doing is
in compliance . It is my opinion that a reasonable approach to this
problem worked out to everyone ' s satisfaction on this item.

RE: Violations - 248 Lafayette Street
Dear Mr. Roy:
In response to a complaint to the Salem Fire Prevention Bureau, on February 11,
1985, in the company of Capt. Robert Turner of Fire Prevention, I inspected your property
located at 248 Lafayette Street, and known as Fairfield Inn, for possible safety code
violations. The results of the inspection are as follows:
1. There is no valid certificate of inspection on file with the Building
Inspector as required by Mass. General Law, specifically Section 108.5.1 of
the Mass. State Building Code, nor has there been one since August 1,
1978. That certificate was issued for eleven (11) rooms not the nineteen
(19) currently in use.
2. There is no building permit on file with the Building Inspector to divide the
basement into rooms for rent.
3. Two (2) acceptable means of egress, as required by the Mass. State
Building Code, are not provided for all rooms on each level. Doors
designated as egress doors were found locked and apparently occupied by
lodgers.
4. Smoke detectors are not provided within the rooms as required.
5. Emergency lighting was not operable on the second level and was lacking in
the number of heads required to properly illuminate the areas required.
6. The basement rooms are not considered habitable due to lack of proper
ceiling height as required by both sanitary and building code.
c 7. The basement boiler room is not within a one ('1) hour rated inclosure, as
required.
In view of the hazards these violation pose to the public safety and welfare, I must
insist that you contact me within forty-eight (48) hours of your receipt of this notice with
plans to make the necessary corrections. Your failure to do so will leave me no
alternative but to instruct the Inspector of Building's to post the building dangerous and
un-safe and to advise the tenant's of their legal rights.
