On February 1, 1992, at the owner's request, I performed a FINAL INSPECTION at the subject property. My involvement in the
project has been limited to the structural design of the building, as it was originally submitted for a Building Permit. My
responsibility has been limited to compliance of the building's structural design relative to the State Buildinq Code.
It should be noted that I was never assigned, or accepted responsibility for any post-permit application professional
services las required by Section 127.2.2 and 127.2,3), or be present on the construction site on any basis to determine that
the work was proceeding in accordance with the documents approved for the building permit.
This report is not intended to certify that the excavations, foundations, dampproofing, structure, and insulation have been
constructed in compliance with the State Building Code. It is my understanding that such inspections were performed and
approved by the Building Official during the normal progress_ of construction.
If it can be assumed that the excavations, foundations, darxpproofing, structure, wiring, plumbing, heating and -insulation,
have been constructed in compliance with the State Building Code, I submit that the building is satisfactorily completed and
ready for occupancy, subject to the following conditions:
1) Remove insulation stuffed into vent at elevator machine room in the basement floor;
2) Eliminate and prevent the storage of any combustible materialsisubstances in mechanicallelectrical ciosetsfrooms;
3) Provide a 36" X 36" platform at the top of the stairs which exit from the basement to the paved area, flush with the
uppermost tread;
4) Renew the elevator certificate which expires February 11, 1992;
5) Install flooring at main floor and second floor office spaces;
6) Remove all scrap materials, debris, tools and equipment from the building;
7) Replace broken window at second floor office space.

I. INTRODUCTION
This report represents results of a test for critical radiant flux
of a floor covering system submitted:
This test, also known as the Flooring Radiant Panel; is identical
to the Federal Test Method Standard Number 372 and the National Fire
Protection Association Test Method Number 253. It measures the critical
radiant flux at flame-out of horizontally mounted complete flooring
systems exposed to a flaming ignition source in a graded radiant heat
energy environment. It is intended to simulate actual conditions that
have been observed and defined in full-scale experiments. The test is
applicable only to floor covering systems which duplicate or simulate
accepted installation practices. Tests on individual components of a
system are of limited value and are not valid for evaluation of the
entire flooring system.
The test results apply only to the specimens tested and are not
necessarily indicative of apparent identical or similar materials. All
test data are on file and available for review by authorized persons.
II. PURPOSE
The Flooring Radiant Panel test measures the level of incident
radiant .heat energy on the floor covering system at the most distant
flame-out point, and is expressed as Critical Radiant Flux in watts per
square centimeter. It provides a basis for estimating one aspect of
flamespread behavior for floor covering systems installed in corridors
or exitways of buildings. The imposed radiant flux simulates thermal
radiation levels likely to impinge on the floor of an exitway or a
corridor whose upper surfaces are heated by flame and/or hot gases from
a fully developed fire in an adjacent room or compartment.
111. TEST PROCEDURE
A minimum of three (3) sets of the floor covering system must be
tested. A specimen measuring 20 centimeters by 100 centimeters is
mounted in a horizontal specimen holder in the test chamber. A gas/air
fueled radiant heat energy panel is mounted in the chamber at a 30°
angle to the horizontal specimen plane. The radiant panel generates an
energy flux distribution ranging along the 100 centimeter length of the
test specimen of approximately 1 .0 watts per square centimeter to a
minimum of 0. 1 watts per square centimeter. The test is initiated by an
open flame from a pilot burner that is brought into contact with the
specimen for 10 minutes. For specimens that continue to burn after the

pilot burner is removed, the test is continued until all flame is out.
The specimen is then removed from the test chamber and the point of
farthest flame front advance measured. The critical radiant flux is
then computed from the Flux Profile Curve determined during calibration
of the test apparatus.

V. TESTING
The critical radiant flux of an inorganic millboard dummy specimen
was checked at the 40 cm mark on the .day of , the test. Critical radiant
flux of 0.5 ± 0.02 watts per square centimeter as measured at a distance
of 40 centimeters from the 0 end of the specimen, indicated proper
operation of the test apparatus. The flooring system was then tested
within parameters outlined in the standard on August 18, 1986.

A hearing on this petition was held August 16, 1989 and continued until October
18, 1989 with the following Board Members present: James Fleming, Chairman;
Messrs. , Febonio, Luzinski, Nutting and Associate Member Labrecque. Notice of the
hearing was sent to abutters and others and notices of the hearing were properly
published in the Salem Evening News in accordance with Massachusetts General Laws
Chapter 40A.
Petitioners, owners of the property, are requesting Variances from lot size, front
yard depth and lot width, so as to permit construction of a professional office
building in this B-1 district.

The Variances which have been requested may be granted upon a finding of the _
Board that: co

a. special conditions and circumstances exist which especially affect the land
building or structure involved and which are not generally affecting other N
lands, buildings and structures in the same district; ca
b. literal enforcement of the provisions of the Zoning Ordinance would involve
substantial hardship, financial or otherwise, to the petitioner;
c. desirable relief may be granted without substantial detriment to the public
good and without nullifying or substantially derogating from the intent of
the district or the purpose of the Ordinance.

The Board of Appeal, after careful consideration of the evidence presented at tho
hearing, and after viewing the plans, makes the following findings of fact: N

1 . The lot in question is irregular in shape, size and configuration, with rn
changes in elevation that make this lot difficult to develop. That
this topography was unique to this parcel and not generally applicable to
other land in the area.
2. The area is a B-1 zone and the use is a permitted use.
3. A Variance for the same structure as presented, which at that time contained
four (4) office units, was previously granted; and because of difficulties
in developing the lot economically, the petitioner has requested an additional
two units in the basement or lower level, for a total of six (6) units.

4. The concerns of the immediate abutter, Mr. Pelletier were addressed and
satisfied by the lowering of the height of the new structure by six (6)
feet so that the the roof ridge line of the new structure will be no more
than eight (8) feet above the ridge line of Mr. Pelletier's building.
5. There will be adequate on site parking.
6. The proposed development is in keeping with the Master Plan of the City.
On the basis of the above findings of fact, and on the evidence presented, the
Board of Appeal concludes as follows:
1 . Special conditions existwhich especially affect the subject property
but not the district generally;
2. Literal enforcement of the provisions of the Zoning Ordinance would work a
substantial hardship on the petitioner;
3. The relief requested can be granted without substantial detriment to the public
good and without nullifying or substantially derogating from the intent of the
district or the purpose of the Ordinance.
Therefore, the Zoning Board of Appeal voted unanimously, 5-0, to grant Variances
from lot area, front yard depth and lot width, so as to permit the construction
of a Professional Office Building containing six (6) units, subject to the
following conditions:
1 . The roof ridge line of the proposed building not be any higher than eight (8)
feet above the roof ridge line of the adjacent Pelletier building.
2. All construction and dimensions be in accordance with plans submitted 10/18/89.
3. All requirements of the Fire Prevention Bureau be strictly adhered to.
4. Construction meet and state and local building codes.
5. A Building Permit and a Certificate of Occupancy be obtained.
6. Any signs be approved by the City of Salem Planning Department.
