Please note change of date This letter was written November 5 not October 5th as the previous copy indicated and was a typo done by me. My letter of February 21,2012,removed the permanent Occupancy Permit for your building. At the same time I issued a Temporary Occupancy Permit to allow the Body shop area to operate and I limited that to day light hours only. A further Structural assessment was done by Strucures North on your behalf which confirmed structural damage to the front portion of the building. It has been about eight months since the fire. To date no repairs have begun. The body shop area is still without required bathrooms and are still operating the office from a temp. trailer.. Due to the concerns about structural conditions, this situation cannot continue into the winter months. Therefore all occupancy in the building is revoked as of November 30, 2012. You are further directed to replace the plywood that secured the openings in your building. It is a two-story metal-framed commercial structure. The building is currently divided into three units that include a attorney's office, an auto body shop and a dog daycare business. The fire took place on the second floor in the office space located towards the front of the building. Following is a general description of the building structure and a summary of our noted conditions and recommendations. Recommendations are shown in italics following noted conditions. For the purposes of this report the front of the building faces west. General Description The roof framing consists of cold-formed steel purlins that span east-west and that are supported on structural steel frames that span north-south, with the exception of the front gable end wall. The front gable.end wall is framed with cold-formed steel studs and channels. A second floor at the front of the building was framed after the initial construction and occupies approximately one third of the building's footprint. The second floor framing consists of a plywood subfloor on open-web steel joists spaced at approximately 16" on-center, supported by wide flange structural steel beams and pipe columns. Noted Conditions and Recommendations The fire damaged portions of the building include portions of the roof framing, front gable end wall, and metal roof decking. Fire fighting efforts also led to water and emergency access damage of doors and windows, as well as to the plywood subfloor of the second floor. The front(first) roof framing bay requires a minimum of seven purlins to be replaced or sistered, five to the north side of the ridge, and two to the south. Eight rafters require replacement or sintering in the second roof framing bay, where paper products stored in an attic space caused the fire to damage the purlins in this area. Of the purlins that require repair or replacement, three are located north of the ridge and five to the south. Portions of the roofing material appeared to be damaged from the fire and will need to be replaced. Please note that portions of the roof beyond the fire-damaged sections may need to be removed and replaced for access, and this should be reviewed with the contractor performing the work. The middle of the front gable end wall requires re-framing or reinforcement. The extent of the re- framing extends from a single column to the right/south of the ridge (that does appears to be in good condition), located approximately 9'-3"away from the ridge, to the column that is located immediately to the left/north of the roof ridge. The latter does require partial-height replacement from the second floor to the roof. Any severely warped members, such as the north column and edge rafters, should be removed and replaced instead of sistered. Warped portions of the front wall metal sheathing will also need to be replaced. All sistering or replacement materials should be like-kind, including any clips/framing angle connections and their components. Fri Playful Paws Doggy Daycare /63 Jefferson Avenue 28 February 2012 Salem, MA Structures North None of the structural steel (frames) appeared to be damaged during the fire. While these members appear to be un-damaged, a closer observation should be made following the cleanup, and/or during the repairs noted above. Several areas of flooring felt soft underfoot, suggesting damage to the subfloor. Plywood decking was visible from the first floor, however it is unknown if the floors have a separate underlayment sheathing between the plywood subfloor and the carpet pad. Following a full cleanup including carpet removal, the full extent of subfloor damage will need to be evaluated and any damaged sections removed and replaced in-kind. Subfloor repair work should occur after the cleanup in order to ensure safe access, and cleanup efforts will need to be performed in a manner that prevents hazards to persons at water damaged floor areas (contractor to provide temporary decking as required, etc.). Roof structure repairs should immediately follow subfloor repairs, with temporary weather protection used to prevent further water damage from precipitation. During our site visit the auto body shop was in near-full operation as this section was largely undamaged by the fire. There is only limited water damage to the front first floor offices, as well as the second floor attorney's office. If office duties are able to be performed off-site, the main area of the Playful Paws portion of the building may be occupied on a limited basis. In the unlikely event of a severe late-winter storm, the front two bays of the building should be evacuated until permanent repairs can be made. Report limitations This report is a summary of readily visible observations conducted during a single site visit to the property. No finishes were removed to expose hidden structure, and no calculations have been performed to determine if the overall building framing or foundations of the structure comply with past or present building codes. This report is strictly limited to structural considerations noted. Egress, guard rails, fire protection, building envelope (damaged windows that require repair/replacement) and other building systems were not reviewed, and they are beyond the scope of this report.
