Our Department has not received any communications or requests for scheduled review of the property as was discussed at our May 7, 2012 site inspection that Fire Prevention was present. Multiple unscheduled site inspections over the past five months of the stairwells have been conducted by this Department, missing life safety equipment and smoke detectors in the common areas were again in violation of the Massachusetts State Building Code, the apparent corrections stated in our May 7, 2012 letter were again non-compliant. This Department showed great restraint and consideration when extending our completion date of all outstanding violations until June 11,2012, due Amy's approaching wedding and the Memorial Day weekend. Ms. Amy Poehler Swenbeck's who was designated youi representative from the beginning of our inspections, stated on May 7,2012 with Lt. Griffin of Salem Fire Prevention and Ms. Elizabeth Gagakis present that she could assure me none of the violations would be corrected by the June 11, 2012, date, she needed a year because of her wedding planning. It appears she was correct in her statement. You are hereby directed to contact this office immediately upon receipt of this notice. Violation corrections need to be completed immediately and an inspection with this office scheduled immediately, additionally all outstanding Municipal tickets need to be paid in full. Failure to do so shall result in the filing of criminal complaints at District Court.
