I was called to do a final inspection for the property located at 48 Howard Street.
While I was there I noticed the water pressure was inadequate for all three units.
Plumbing Code Principle 42 Adequate Water Required.
Tests were taken with all fixtures running. The Is' floor had good pressure, 2nd floor less
pressure and 3`d floor minimum pressure.
This problem must be fixed and reinspected before a Certificate of Occupancy can be
issued. .

Street is currently occupied. This is a violation Massachusetts General Laws, and Salem City
Ordinances.
1. There are open Building Permits for this property.
2. No Certificate of Occupancy has been issued for this property by the Building
Department.
3. No Certificate of Fitness has been issued for this property by the Health Department.
You must contact the Salem Building Department by close of business July 7`h, 2014. Failure to
do so may result in municipal fines and further enforcement action.
If you feel you are aggrieved by this order,you have the right to appeal to the Massachusetts

An inspection of your property was conducted on February 3, 1999 and the following

3. Front step (a couple of bricks broken and some loose boards on wooden part).

Dear Mr. Kerrigan:
Upon request of the Health Department an inspection of your
property at 46-48 Howard Street was conducted. The following
violations were found and must be corrected.
111) The property is a legal (4) four family dwelling and must
be maintened as such. We have reason to believe that you
are renting a fifth unit .
112) Handrails must be installed at exterior front stairs .
113) Stairway from 2nd to 3rd floor in front stairwell must have
closed risers , and ballasters no more than four apart .
li4) Pole 4"x6" should be installed at third floor stairwell to make
safe under side of bearing.
#5) All apartments must contain two seperate means of egress.
116) Rear exterior stairway does not conform to code. Ballasters
on rails must not he any further than nine (9") on center.
Rail at third floor must be 42" in height.
117) Gas grill must be removed from Ist floor porch.
118) Section of gutter should be replaced on west side of building.
119) Fence in driveway should be made secure.
