Dear Ms. Watson :
Thais is to advise you that members of my staff have reviewed your preliminary
submission requesting permission to construct commercial driveways and a
drainage connection on Highland Avenue in the City of Salem.
Please be advised that before we can recommend issuance of this permit by
our Boston Office the following items must he addressed by your Consultant.
DRAINAGE
1. Your Consultant must submit a drainage analysis for our entire
system including the outfall area.
2. The contours that appear on the plan appear to be plotted incorrect—
ly and the drainage areas are not shown correctly nor completely.
GEOMETRIC DESIGN
1. The curb radii on the entrance drive right hand corner should be
reduced from a forty—five (45) foot radius to a thirty (30) foot radius.
MISCELLANEOUS
1. Pavement markings on the Highland Avenue roadway shall all be plotted
on your submission.
2. All property lines shall be identified on the plan.
3. The Massachusetts State Highway Baseline shall appear per Department
Standards.

You were advised that Permits for utility connections shall be issued only
to the respective owners of said. utilities.
Therefore, the City of Salem will be the Grantee for an water or sewer
connection servicing this establishment.
I am recommending that all further inquiries on this matter be directed
to the District Permits Eneineer, Mr. Fred Harney .

Dear Ms. Watson:
I am returning your Permit Application along with the Plan and Traffic Anaylsis which was
submitted to us on November 15, 1988 for the proposed Kentucky Fried Chicken Restaurant
which will be located on Highland Avenue in the City of Salem.
Please be advised that the District does not intend to proceed further on this request
as your Consultant, Mr. Phineas Alpers, has again failed to submit an adequate plan
for our review.
I have been informed by my Permits Engineer that Mr. Alpers should be familiar with
the engineering data which is to appear on all plans when submitted to this Office.
The Department has put together a traffic study guide outlining the information we
require. Mr. Alpers is in receipt of this guide and he must follow it to the letter
as I do not intend to have my Staff waste their time on inaccurate submittals.
I am enclosing a copy of this guide for your convenience.
I have also bee;i informed that members of my Staff have met on four (4) different
occasions and with four (4) different Consultant Engineers concerning the proposed
drainage tie in which you will be requesting. Even though my Staff is here to serve
the Public we believe that we have given a more than adequate amount of time to each
of them on this matter.
As for your request for us to notify the Salem Building Inspector that he may proceed
with the issuance of a building permit, we cannot make this recommendation until this
matter has been resolved.
