Dear Mr. Monroe,
This letter is to inform you that we have made preliminary
inspection of the public spaces in the Medical Office Building at
One Hundred Highland Avenue in Salem.
We find that the buildiing has been constructed substantially in
accordance with the plans and specifications provided by this
office and in accordance with the Building Code of the
Commonwealth of Massachusetts and the requirements of the City
of Salem. Please note that there were 48 items which the owmer
chose to delete from our original drawings in his negotiations
with the contractor. These included the deletion of the first
floor toilets. A copy of all 48 items is attached.

3 . Foundation drainage shall be as detailed on
the drawings, however , shall extend only N/A
around the perimeter of the habitable basement
areas, as well as under the floor slab of the habitable
basement as shown on the drawings by Barlow &
Gilfillen,Architects .
4 . Curb at retaining wall . The concrete bumper
curb shown on typical retaining wall detail SDI
SD - 2 is deleted.
5 . Retaining wall . The retaining wall North-
westerly side of the site, as detailed in SDI
typical retaining wall section Sheet # SD-2
will be deleted. In its stead will be an
allowance of $2 ,000 .00 to cover the costs
of whatever work may be required.
6 . Crawlspace slab . The slab in the crawlspace
as well as vapor barrier and mesh- shall be Al
deleted . The crawl space shall be left in a
rough graded manner and covered with 4" of
gravel .
7 . Delete 1 door @ storage. This item involves
the deletion of one door as well as accompanying AI, H1
frame and hardware into an area of the basement
entitled basement .
Crawlspace locations as L31iown un urawings H 1
and E 2 . In general , the items to be deleted
will be the furnishing and installation and H1 - Crawl Space
exhaust fans (noted as EF 2 ) , ductwork , louvers,
vented with duct-
unit heaters, (noted as UH 1 ) , humidistats, ing through
miscellaneous hangers, wiring and area ways as Basement
shown on typical section entitled 'area way. '
9 . Area ways delete 4 and change 2
This provision includes the installation
of metal area way retainers as normally H1
utilized in residential construction for the
use with the exhaust fans for the habitable
basement area . Both of these would be located
along S line. Other areaways have been deleted
in the preceeding item.
10 . wall bumpers. Delete all oak wall bumpers as
shown on drawing A 16 as well as all backing for the A16
bumpers.
11 . Eliminate 1st floor toilet and Janitors
closet. Delete in its entirety the first floor A2•P2
toilet and janitors closet. This change A14
includes the deletion of all plumbing, electrical
drywall , flooring and ceiling as specified
for those two rooms . The space shall be
finished with carpet flooring and continuation
of a 2 x 2 ceiling as shown on the
reflected ceiling plan . Lighting shall be provided
with 2 pcs. of 2 x 2 drop in lights . Fixtures shall be
included within the lighting allowance.
12 . Eliminate canopy. This change includes the
elimination of the canopy in its entirety with A14
the exception of the caissons for the canopy
which have already been installed.
13 . Substitute Conc. for tile @ Plaza
This change incorporates the utilization A14
of broom finish concrete sidewalks at the
entry to the building and the deletion of
all references to tile in that area. The
area approximately 18 ' square which forms
the corner of the building shall
receive no concrete and shall be left to be
finished with landscaping. The cost of the
landscaping shall be included within land-
scape allowance.
14 . Substitute lobby tile w/carpet
This change includes the installation of A14
carpet along the main corridor on the
first floor . The carpet strip shall be
approximately 6 ' 9" wide . The tile shall
only be included as shown on 1/4 " first
floor plan drawing A 14 between column lines
"l C" and '2' and "AC" and "B" .
Substitution for yravel stop shall be allowed . Hickman style
used.
16 . Eliminate mechanical roof screen
This item includes the elimination
of mechanical roof screen in its entirety A5
including all penetrations for roof.
17 . Slope steel for drainage @ roof A4
This item will not be utilized. Pitch for
drainage shall be the minimum allowed by the
roofing manufacturer and shall be accomplished
by utilization of tapered insulation.
The tapered insulation shall meet the require-
ments of the State Building Code.
18 . Substitute asphalt shingles for standing A4
seam metal roof. This item allows for the
substitution of 215 lb. fiberglass asphalt
shingles in lieu of the standing seam metal
roof .
19 . Substitute fir for cyprus. This item A5
allows the substitution of fir for cyprus on
exterior woodwork .
20 . Substitute Kane Gonic brick w/$325 allowance
This item allows a $325 . 00 per 1 ,000 A5
allowance for the purchase of a standard
size brick to, be selected by the architect .
The allowance cost includes brick purchased
f .o.b . job site and all sales tax .
21 . Substitute Cronolite
This estimate is based upon utilization Al2
of windows manufactured by Closure Co.
of .Woburn, Mass . , or equal .
22 . Drapery Pocket ( incl . decrease in window size ) H1, H2
This item provides for the deletion of Al2
the drapery pocket as shown in details
H 1 , H 2, and H 3 on A-12 and also allows
for the reduction in height by approximately
15 " of all exterior windows . This will
allow for the ceiling to run directly into the
top of the window frame as the drywall presently
shows doing in detail H-1 on A 12 .
23 . Change plywood to gyp.sheathing
Delete all references to utilization of N/A
1/2' CDX plywood sheathing and utilize
1/2' exterior grade gypsum sheathing in
its place . 1/2 ' CDX plywood shall be
utilized in areas with wood siding .
24 . Eliminate extended head at window and
substitute for breakmetal column closure . N/A
All breakmetal in the project shall be
replaced with the appropriate adjacent exterior
finish material . This being either brick or

on drawing A 12, H 2 on drawing A 12, and S2 on A 12,- 53 on
drawing A 12 and H3 on drawing A 12 shall be deleted.
Windows shall be mounted directly to 6 ' metal studs .
28 . Simplify handrail (mill work ) sq . oak
Detail E /A 13 shown for wood rail section shall A13
be revised. New detail shall be the same size oak
with easen corners but without the 1/2" rabbit . Fastening
of the oak shall be to a flat plate recessed into the
bottom of the oak by approximately 1/2" and there shall
be no wood plug in-fill on the bottom of the oak .
29 . Simplify Detail H on A 13 .
At the atrium rail , A13
detail shall be changed as follows :
1 . the channel iron and 20 gauge closure
shall be deleted.
2 . concrete floors shall extend out to within
1/2" of the face of the gypsum wall board.
3. the gypsum wall board shall run up to the
top of the concrete floor .
4 . the railing shall have a piece of flat stock
on the bottom and shall be attached to the
concrete floor expansion bolts .
5 . Stairs - 20 gauge closure piece wrapping the
channel shall be deleted and the channel
painted to match the stairs .

This item includes the change of SD1

deleted and landscaping graded down to the
edge of the bituminous concrete.

concrete splash box or extend down with the use of PVC

in the allowance only.
Additional clarifications: (within contract documents )
Allowance for the tenant finishes for the 3 suites presently contemplated
is included in this price. The allowance includes all costs including
the general conditions and fees associated with the completion of the
tenant finishes .
The contract is based upon the deletion of supplementary and special
conditions as provided us by Barlow & Gilfillen in their original specificatii
contract .
This contract sum is based upon the requirement for a general bond
as provided in the contract documents provided by Barlow &
Gilfillen being waived . Should a general performance and
payment bond be required, there shall be an additional cost to the
project .
Pricing is based upon the ability of The Welch Group to
utilize all onsite fill in any backfilling operations required on
the site .
The contract does not include any provisions for dealing with any hazardous wast
or materials which may be encountered during the course of construction.
This contract does not include the cost for testing and any additional
architectural/engineering services which may be required.
This Contract does not include the cost of Gas Company or Utility Company
backcharges.
Builders Risk Insurance is to be provided and paid for by the owner .
III . Construction and Build-Out to Completion of Owner O__f_£_i__c_e
Suites
The Guaranteed Maximum Price includes only the work and materi-
als necessary to partition the three ( 3 ) owner suites off from the
common areas, but includes no interior partitions, or walls, electri-
cal , plumbing, or lighting, finished floors, ceilings or any other
work or finishes to the Owner suites except as may be provided within
a total allowance of One Hundred Thousand Dollars for the build-out
and finish of the Owner suites, which allowance amount is included in
the Guaranteed Maximum Price .

The Radiation Control Program has reviewed a shielding design con-
sisting of floor plans, workload information, occupancy information,
and use factors.
The Radiation Control Program is of the opinion that the submitted
shielding design is drawn in conformance with modern radiological
health practice and hereby approves it. .
Compliance with 105 CMR 120 . the Department ' s Radiation Control rules
and regulations, require in sub-chapter 120 .103 (b) and (c) that each
person that intends to maintain a facility or acquire a source of
ionizing radiation shall apply to the Massachusetts Department of
Public Health ' s Radiation Control Program to register the facility
prior to acquiring , installing or commencing to maintain the facility.
The enclosed "Application For Registration Of Ionizing Installations"
shall be filed with the Massachusetts Department of Public Health' s
Radiation Control Program according to the regulations outlined above.

Be it ordained by the City Council of the City of Salem, as follows:
Chapter 7 , Building Code of Salem Code of Ordinances is hereby amended
�rdinn 1. by adding the following:
Section 7-C. Development Inspection Fees
Ordinances, as amended, with regard to the construction, reconstrucii.on,
or rehabilitation of any structure or building, the following
requirements and fees shall apply:
(1) All residential developments, either new construction, r;.
reconstruction, or rehabilitation, of six (6) or more dwell-in.-
units ,
wellingunits , and commercial developments, either new construction,
reconstruction, or rehabilitation, of more than 10,000 square
feet , will be required to pay for inspection services for the
following:

(2) The City, through its Building Department, shall invoice the
developer on a monthly basis for inspection services at the rare
of $20 per hour.

Dear Dr. Curtin,
We have recieved the UTS reports numbers 8, 12, 13 and 14
regarding the changes from the required 4000 psi concrete to
3000 psi concrete. After careful review with Brask and Standley
Engineers we do not find the structural integrity of the
foundations to be compromised by this change.
Our letter of July 8, 1987 recommended the withholding of
$5,384.47 from application number 6 and $5,201.00 from
application number 7. We no longer take exception to these
nand recommend funds to be released.

1. Building Inspector, Steve Santry, and I reviewed site, roof
to basement.
2. Issues relating to the rating above elevator were raised and

3. Masons were laying brick an east and north walls.
4. There was extensive sweating of copper water lines in
basement. This should be reviewed.
5. Building Inspector has requested copies of both UTS reports
and structural engineer's reports/letters.
6. There was temporary lighting in the basement.
7. Structural engineer has been called regarding the welding of
baseplates on columns. Structural engineer informs me that UTS
report is to certify the welding. Architect and Building
Inspector are waiting for these reports.

8. The Building Inspector wanted to know why the grout was used
(3"-4") at beams instead of bearing plates as shown on the
drawings. Contractor should respond directly to Building
Inspector with copy to architect. Structural engineer will

Grout is beginning to crack and there could be a softening
issue that the contractor MUST REVIEW IMMEDIATELY !
9. Contractor must call Building Inspector before walls are
closed for inspection.
10. Owner needs quote immediately on the price of the two (2)
skylights for the second floor offices.

Observation:
Steel studs at the windows were being placed. I called to Norman
the variations to approved sketches. He informed me that work
would be corrected.
We are still waiting for a superintendent for the project.
There appears to be no concern regarding resolving any of the
waterstop issues in the final sections of formwork.
Bob Buthman needs final answer on the fire rating above the entry
area. He discussed this with Mike Keane late afternoon. Mike
informed him that a two hour rating is necessary. Bob wishes to
change the system we designed. We are waiting sketches of his
proposed change for review.
Norm Welch discussed a variation is the dimensions between the
head of the windows and the head of the door. There is
approximately a two inch difference. I approved modifying the
glass transoms to keep both consistent. There is to be no change
to the job cost.
Welch need final suite drawings from the Architect. Norm has
assured me that there are no dimensional changes between the
original drawings and what was built. The Architect is proceeding
on the basis. Bob Buthman has indicated that he will double check
the dimensions and advise the Architect if there are any
variations.
There was no meeting last week.
There should be ladders that make all levels of the building
accessable.

Dear Bob,
This letter is to follow up the site visit to 100 Highland Park
of the Building Inspector on Tuesday and my follow-up
conversation with him on Thursday, September 10, 1987.
1. The ceiling over the entire lobby area will require a two-
hour rating.
2. He and I concur on the roof area that a pressure-treated
wood should have been used. However, there is no code issue here
and the changing of the work is at the option of the doctors.
3. He recommended that the concrete slab (rat slab) be poured
in the crawl space. This was shown on our original. drawings but
later deleted.
He advised me that the humistat and the ventilation must be
provided as per Section 503.3 of the Code. If Welch intends to
delete his, then you must discuss this change order with the
doctors. It must be provided for.
4. The building inspector wants samples of all carpets and pads
chosen. Further, he wants the normal flame spread and
specification data.
5. The UTS reports are being forwarded to the Building
Inspector. He is specifically concerned about the -1 ding of the
steel. According to the structural engineer, we ari still
waiting for the reports from UTS certifying that t. welding and
connections for the steel meet the requirements of. drawing.
6. He was concerned regarding the amount of grout (3"-4") used
on the plates instead of the sill plates required by the
drawings. Brask and Stanley is reviewing this and will advise.

7. The Inspector suggested that a "smoke hatch" be used at the
roof. I informed him that the "Bilco" was bid and had been
approved. Further, I agreed that a smoke hatch should have been
considered but that budget was a primary concern.
I believe that this fairly expresses the concerns of the Building
Inspector. He has requested copies of letters from our
structural engineers regarding the foundation and steel as he
expressed many of the same concerns we have raised before.

After a review of the corrected test cylinder reports 1112, 1113 and 1114,
we have concluded that these reports are satisfactory; the strength is
at the range the job specifications require.
The reason for the confusion was a relatively new employee broke the
original cylinders that did not belong to the above referenced project.
The cylinders for this project were found and broken and, as a result,
in the change of data, corrected reports were issued. This was a freak
mixup that should never have happened, but when handling 300 to 400
test specimens per day, an occasional mixup can occur.

Dear Dr. Curtin,
Enclosed are the revised details for the head and sill at the
windows. We have revised these drawings as per discussions with
Welch regarding cost savings. We have removed the steel channels
above and below the windows and replaced them with a system 'of
fire retardent wood framing, braced back to the underside of the
metal joists.
We are concerned that this change may have some long term
maintenance problems because of additional deflection to the
windows.
Further we are concerned regarding the aesthetics of how the
brick sill will look after the change. We will work closely with
Welch to do what we can.

Dear Sir :
Your letter of July 14 , 1987 requesting certification of the
referenced project 's work , to date, and outlining by enclosure
(dated July 13, 1987 ) observation of allegedly unacceptable
work has been received by this office.
To the point of certifications , this company 's statement is
made within every requisition as a basis of payment, and is
identified on the front page of the A. I .A. Requisition Form.
Further certifications, as well as replacement of your engineer
by the undersigned as "engineer of record for the work " would
infringe upon professional ethics and would be considered
unorthodox and unwarranted .
Your enclosure (July 13, 1987 ) of allegedly unacceptable work
has been carefully reviewed and is answerable categorically by
the following:
1 . Dewatering was successfully completed on this project
using continuous pumping methods, supplemented by a
crushed stone drainage bed atop of ledge and/or
hard-bottomed material . All concrete and backfill was
placed "in the dry" in accordance with proper practices .
2 . Caisson C-16 is located , by plan, approximately three ( 3)
feet from the building cellar foundation line .
Obviously, a small portion of its circumference would be
exposed as the foundation was constructed. Because its
base is slightly below that of the nearby footings , no
settlement was recorded. Further , no lateral movement
was detected .

3 . As all heavy equipment employs either wheels or pads, no
meaningful vibrations would be transmitted to caissons .
The minor amount of caisson reinforcing steel which was
slightly bent over the course of site clearing, upon
straightening, does not effect the structural integrity
of the building.
4. In order to insure the workmens safety, the foundation
backfilling had to be completed to certain minimum
heights , as the future building work could not proceed
over the 14 foot chasm existing. Final backfilling was
completed after the installation of the concrete deck
5. Engineering soil tests as well as owners designation
confirms proper use of material .
6 . Reportedly, the alleged improperly installed waterstop at
the top of foundation wall and grade beams is concerned
with the skip of the waterstop at the concrete columns .
As the columns base plate locations obviously conflicted
and prevented the continuous waterstop installation, the
proper priorities were given to the column base plate
location and installation .
7. The waterstop at the basement from the foundation wall
into the floor slab was installed correctly. The
apparent confusion stems from concrete laitance
encroachment on the face of the outstanding waterstop
legs giving it the appearance of departing from the
straight line installation. This situation is very
normal in waterstop installation, and was proven correct
as the waterstop sides were chipped away from the
concrete laitance and folded down into position for the
basement slab.
8. The missing reinforcing conditions at grade beam pockets
were installed, as discussed previously at a job meeting,
ie. , any missing reinforcing was drilled into the sides
of the receiving pocket and the reinforcing bars
positioned prior to the intersecting grade beam placement .

9. The misconception of construction field ability resulted
in later required chipping of grade beam pockets . Forming
out pockets is done with styrofoam, which in this case
due to both the large pocket required, as well as,
principally the interference of a myriad of reinforcing
of the main wall, as well as stirrups and anchor bolts at
the column locations , prevented any sensible exactness of
pocket forming.
10 . This condition is not located and is unknown to us . We
suspect this condition is isolated in occurrence .
11 . All situations of honeycombing will be corrected by an
overlay of concrete patching. Stone pockets have not
been found in our investigation.
12. Form stripping has only influence on the final strength
of concrete and only under specific conditions . Although
some few forms were stripped quickly, no lateral loads
were allowed before full curing, and no diminishment of
structural abilities occurred .
13 . The only occurrence of anchor bolt misalignment was
located at 2-Cb, which was shown, to your structural
engineer, without ensuing comment, as one of the four
anchor bolts did not engage.
14. The vertical reinforcement was placed and cast in
concrete, although not shown on approved reinforcing
plans, but resulted from a verbal request of the
structural engineer .
15. No caisson exists at C-2 .
16 . Plate washers were not assembled with anchor bolts due to
their conflict with the stirrup column reinforcing, as
was discussed with the engineer. It is understood there
are no structural considerations from the required
deviation.

18 . 4000 p.s. i . concrete exists in areas as called for .
19 . Anchor bolts have been both welded and nut and washer
cinched, as shown to the structural engineer and as
verified by testing inspectors. The engineer 's have
acknowledged no disapproval for this normal field
practice.

I believe this should clarify any questions on field
construction conditions required by practical construction
procedures and/or without detriment to the function and
integrity of the structure .

Welch (roup. Please note that application #6 was not received until after we had received V.
At this time I cannot certify that the quality of the work is in accordance with the contract
doaments. Please review our previous field notes and the attached list. This decision was
previously comm ncated to Chuck Stone on Friday and follows my meeting with Bill Quinn and
Patrick Ckrtain.
To resolve this we propose the following:
1. That your engineer (ie Norman Welch) certify that the work through these requisitions and
including the work relating to the welding of the anchor bolts to the base plates will be
sufficient to meet the requirements of all later work.
2. Further the Welch Group agrees to indemnify Barlow and G- lfillen, Architects (including their
consultants and employees) from any costs, losses, damages including attorneys fees arising
from any issue pertaining to the above work covered.

The following is a list of observations made by Barlow and
Gilfillen Architects, Brask and Standley Engineering and /or
Ralph Camerlengo which do not comply with the contract documents
and have been found to be unacceptable.
1. Inadequate dewatering during excavation, casting concrete and
backfilling.
2. Partial exposure of caisson C-16 during basement excavation.
3. Burying and running heavy equipment over caissons causing
unnessasary vibrations in caissons and reinforcing steel to be
bent and twisted extensively.
4. Backfilling foundation prior to securing concrete deck.
5. Use of on-site material in backfilling basement slab.
6. Improperly installed waterstop at the top of foundation walls
and grade beams.
7. Improperly installed waterstop at basement slab.
8. Method of correcting missing reinforcing conditions at grade
beam pockets has never been reported , approved or confirmed.
9. Grade beam pockets misplaced or inadequate requiring extensive
chipping.
10. Inadequate concrete cover on vertical reinforcement.
11. Extensive honeycombing, and stone pockets in concrete.
12. Form stripping prior .to recommended concrete curing period.
13. Improper alignment of anchor bolts.
14. Vertical reinforcing between interior piers and cast-in place
concrete beams (B1 and B2) was not cast in concrete.
15. Reinforcing steel at caisson C-2 not embedded in concrete
pier. Several bars cut off at top of caisson.
16. Plate washers missing from anchor bolts.
17. Chipping of concrete at slab to relieve waterstop from
concrete wall.
18. Use of 3000 psi concrete in lieu of 4000 psi concrete.
. 19 Welding anchor bolts to column baseplates.
20. Misalignment of pier and column at CB2. Baseplate overhangs
concrete approximately 4".

referenced project. As a result of numerous areas of the work
which do not conform with the Contract Documents as identified
by this office, Brask and Standley Engineers and Mr Ralph
Camerlengo we find it necessary to make several adjustments to
the certified amount on these applications.
Below is an outline explanation of these adjustments and our
estimated value of each item.

be corrected at the contractor's expense.

Information•
The second floor decking which was to have been galvinized using
ASTM A525, G90, 0.90 Oz/sq.ft. has been galvinized using G60,
0.60 Oz/sqft. This is in conflict with the specifications.
The roof decking delivered was not galvinized. We advised The
Welch Group that the roof deck is to be galvinized per the
specifications.
Chuck Stone agreed to The Welch Group installing vapor barrier
in the crawlspaces.
Discussed with Highland Realty Trust and The Welch Group UTS
-eports indicating that 3000 psi concrete had been used

Erection of'structural steel at the core roof.
Observations•
The grout under several column baseplates has cracked and
loosened, this condition is to be corrected prior to the final
tightening of the nuts on the anchor bolts.
As reported in earlier reports many anchor bolts were cast too
high. We instucted the contractor that these bolts were to be
cut and rethreaded in order to properly accept the nuts. Instead
the contractor shimmed the nuts with numerous washers and plug
welded the nuts to the bolts.
In several locations where the bolts were cast too low the
contractor has plug welded washers or nuts to the anchor. bolts.
We noted several locations (at the steel located below the first
floor slab) where the anchor bolts are missing and that the nuts
are missing. These locations were reviewed .with John Hansbury
and are to be corrected.
Columns Cb-1 and Cb-2, do not align with the piers below. The
baseplate Cb-1 falls within 1/2" of the edge of the concrete
pier. The baseplate at Cb-2 overhangs the pier by 3". This
condition is unacceptable.
The alignment of the anchor bolts and the baseplate with pier
Ab-3b has been corrected.
Basement slab control joints have been cut.
Information•
The welding of the moment plates to the columns is to be
contiuous as called for on the drawings. The gap between the two
angles is to be filled soild with weld or a piece of plate when

installing the backer bar to insure that the moment weld will be
continuous.

Requested that Bob Buthmann subbmit a price from his window-
supplier to add 8" extended heads over the windows to the

Observations
Excessive amounts of debris were present in the formwork for the
top pour.
2 X 4 key form had been left in the key approx. 4' -011 along Col
A at Col. A5.
Standing water present on the basement slab.
Moment welds at Col E2 second floor and roof framing has been
burned through top and bottom.
Moment weld at Col. E1 Burned through at top.
Moment weld at Cbl is incomplete.
Roof framing welds were not complete.
Large amount of soil has been brought in from off - site and is
being stored on. the south end of the site.
Many r-f the nuts are still missing from the anchor bolts as
noted in earlier reoorts.
Information
The contractor was notifed to clean out forms before pouring
the concrete.
Moment welds which have been cut or were incomplete were brough
to the contractor' s attention. These plates are to be properly
replaced and moment welds completed.
Note: These field observations DO NOT constitute inspection
of the welds. As advised last week we recommend a
visual inspection of the welds with soot testing
thaughout the project
The sample panel of brick has been delivered to the site and is
ready for review by the architect and owner.
Second floor slab reinforcing is to be 4 X 4 W2. 9 X W2. 9 as
directed in the Architects Field Report # 04 May 05, 1987

Informed Bob Buthmann that a review of the domestic water needs
indicate that a 2" main may not be sufficient to handle
speculative loads similar to those required by the present
tennents. We recommend that the main be increased to 3" or.
larger with the approval of the City of Salem.

Observations
Second pour to lock in concrete deck has been cornpieted with the
exception of the southwest corner of the core. Forms have been
stripped and with the exception of some minor honeycomoing the
workmanship appears good.
Large stones (up to 24" dia. ) have been placed against the
foundation walls during the most recent backfilling and grading.
The protection material has been extensively damaged.

lobby be balanced off by using either two #4 or two #5 windows-
at the corner clerestory.

Drilling and chipping concrete foundations at grade beam seats.
Erection of construction sign at northeast cornner of the site.
Observations
Backfilling operation is complete to approximatly 12" of the
underside of the slab 'within the basement. The perimeter of the
building is backfilled to an average of 30" from the top of the
lower pour of the foundation wall.
The concrete pier Ab,3b in the basement was poured out of place
causing the baseplate and anchor bolts to be drastically
off-center.
The vertical reinforcing steel around the entire perimeter of
the foundation has been placed too close to the outside face of
the wall. As a result most of the bars do not have minimum 1 1/2"
of concrete cover (as specified in Section 03300) and numerous
bars have as little as 1/2" cover. Some spalling around the top
of these bars is evident.
The vertical reinforcing along Col. line 3 and Col. line lc will
not allow for the 4" seat required for the placement of the
Span-Crete deck.
Waterstop splicing at the top of the wall at piers and pour stops
is unacceptable. In all instances of splicing at the piers the
waterstop was simply cut off at or below the bead and rested on
top of the wall rendering it useless. Where fusing was attempted,
the fuse was sloppy and incomplete.
Anchor bolts at various locations around the building are not
properly aligned or vertical. The contractor is reminded that
burning of the holes to increase their size is not allowed under
Section 05100.
The keyway at the top of the wall between Cols. Aa,lc and Ac,lc
extends only 48" south of Col Ac,lc.
Confirmed report by Ralph Camerlingo (4/29) that the basement
slab waterstop deviates as much as 12" from it's horizontal
plane. Further observed that in the area of Col. B,3 along the
south foundation wall, approximatly 6'-0" of the basement slab
waterstop had changed from the white slab type to the black type
use at the top of the wall and the waterstop is not embedded in
the wall.
Vertical reinforcing at the piers at Cb,lb and Ca,3a were not
installed. (See Drawing S-5; "TYPICAL INTERIOR PIER AT CASSION,
SECTION .15-15, and FOOTING and PIER SCHEDULE)
Large stone pocket (approx 24" sq) located between the crawlspace
access openings.

Reinforcing steel for Grade Beams 6,7,9,10 and 11 has been
erected.Informed the contractor that the top steel in grade beam
6 is improperly configured.(only 1 #6 bar was placed at the top
of the wall. )
Informaton•

The structural engineer has determined that the misalignment of
the column baseplate at Ab,3b is unacceptable and will have to
be corrected as soon as possible. The concrete pier at this {
location will have to be removed and replaced in the proper
alignment.
Prior to starting any repair work the contractor is to submit
detailed information on proposed methods of correcting the work
to comply with the standards set forth in the Contract Documents
for the review by the engineer. Such information will include but
shall not be limited to the following:
s
- repair or replacing all honeycombing, stone pockets, cold
joints and damage to the concrete in compliance with the
standards set forth in the specifications.
- limiting the potential damage caused by the insufficiant
cover of the reinforcing steel.
- splicing and fusing of the waterstop to make a continous
moisture barrier between the upper and lower foundation wall
pours and between the basement slab and the foundation wall.
- method of seating grade beams and anchoring reinforcing
steel in locations where no pocket was provided or bars are
missing.
The contractor will be required to have all repair operations
inspected while repairs are being made and certify that all
repairs sufficiantly meet the requirments of the Contract
Documents.
To date, the Architect has not recieved reinforcing inspection
reports covering all the work that has been done.
Final report on soils test of the backfill material is expected
soon.

Forms being stripped from "B-2" and grade beams "GB-6,7,8,9,10 &
11.
Chipping concrete foundation along "lc" line for grade beam
pockets for "GB-12" and "GB-13".
Grading and backfilling in crawlspaces south of "B-2" and along
southern perimeter of foundations.
Observations
Form stripping underway was completed within minutes of my
arrival leaving all forms removed less than 18 hours from the
time the concrete had been placed.
"B-2" and "GB-11" were poured without the engineer receiving
details from the contractor regarding his proposed method of
correcting missing reinforcing which tie the beams into the
foundation wall.
"B-2" was poured without details from the contractor regarding
his proposed method of correcting missing reinforcing situation
at pier "3aCa". Rods were omitted by the Contractor in the
original pier pour.
The end of grade beam "GB-6" was cast approximately 10" to 12"
from the northern edge of Caisson "C-4". Contractor had been
instructed to pour "GB-6" to the face of the pier.
Corner steel reinforcing bars tying grade beam "GB-5" to "GB-6"
have apparently been left out.
The following pockets in the foundation wall do not have all the
proper reinforcing bars located in the proper areas to accept
the main steel from the grade beams and also the #4 bars which
are to be located at 12" O.C. appear to be missing.
On the "lc" line On the "2" line
1. "Ac-lc" (GB-1) 1. "C-2" (GB-8)
2. "Ac.5-1c" (GB-13)
3. "B-lc" (GB-12)
4. "C-lc" (GB-3)
Concrete chipping by jackhammer has partially exposed an anchor
bolt at "Ac-lc". Engineer recommends further chipping of
concrete to more fully exposed bolt such that it will become
fully embedding in concrete poured at "GB-1".
Extensive jacking out of grade beam pockets is evident throughout
the foundation and should not have been required.
Water stops placed in the top of grade beam "GB-6,7,8,9,10 & 11"
consists of many short lengths and are discontinuous. No attempt
was made to fuse the joints. Water stop varies from 6" to 9" at
various points along the grade beams for no apparent reason.
Some areas of water stop do not align properly.
Heavy equipment used in grading operation is being continually
run over remaining caisson C-1, C-2, C-3 and C-17. Reinforcing
rods have been bent and twisted and caissons were visibly
disturbed by the equipment.
Vertical reinforcing steel in all walls are bowed somewhat
out of alignment (not in the same plane).
UTS on site to take soil sample for proctor analysis.
Information•
A meeting was held in the job trailer following site inspection
by the Architect and Engineer to discuss on going problems with
the quality of work and compliance with contract documents (see
meeting minutes attached).
Soil report containing certifying that on-site fill complies
with specifications has not been received. The Welch Group now
informs us that report will not be forth coming because soil
will not meet sieve analysis requirements.
Forms had been stripped from "B-2", GB-6,7,8,9,10 & 11 much
sooner than the minimum of three (3) days. Norm Welch directed
crew to temporarily support "B-2" until further curing could be
achieved.
Details of contractors proposed method of correcting work have

The Welch Group was again informed that the offset base plate
condition at Ab3c is unacceptable. The pier and footing are to
be removed and relocated in their proper location as stated in
Field report # 09.
Deck Shop drawings and plumbing shop drawings were returned to
contractor without review. Submitted copies were insufficient
for proper distribution.
Dr. Nersasian and Dr. Monaldo reviewed and accepted brick layout
sketches. One copy was given to The Welch Group and one retained
by the Architect for record.
Steel deck galvanizing shall be G-90 as specified in Section
05100, Part 2, 2.01, F para. 3.
Drawing Clarification
The contractor was informed that the expansion joint shown at
the perimeter of the basement slab on drawings A-7 and A-8 is not
required. Refer to structural drawings S-3 and S-4 for the
proper detail for this condition.

Meeting was called in order to discuss ongoing problems in the
quality of work and compliance with Contract Documents relating
to foundation work.
1. Two week delay on receipt of UTS report on suitability
on-site as backfill.
o Following a telephone conversation with UTS Norman Welch
informed us that soil report will stand as is until
further testing can be done.
Mr. Welch acknowledges soil will probably not meet sieve
analysis requirements in specifications however his
opinion is that the material can be suitably compacted
to serve as fill under the slab.
Mr. Welch pointed out the Agreement between the Owner
and Construction Manager contains a qualification of the
price regarding the use of on-site material.
o Joe Standley restated his position that the material
will not meet the requirements of the specifications and
is unacceptable as fill material (refer to M. Keane
letter to Dr. Curtin dd March 31, 1987). He will not
accept the bearing value of the soil as a qualifying
property for the use of the material. It must meet
sieve and compaction criteria as set forth in the
specifications.
Statler Gilfillen to establish meeting with Joe Standley
and Norm Welch to resolve issue and advise owner of a
course of action.
The Welch Group will arrange UTS to conduct Proctor and
Compaction Tests on the soil as soon as possible.
2. Stripping forms less than required three (3) days.
o Norm Welch was "surprised" to see forms stripped so
soon. He has ordered B-2 to be supported. He does not
feel structured or aesthetic damage will result.
3. Discussed condition at C-4.
o Joe Standley pointed out that GB-6 does not extend to
the face of pier as instructed and corner steel was
missing.
4. Exposed Anchor Bolt at Acic.
o Joe Standley advised contractor to chip addition concrete
from around bolt to allow bolt to be embedded in upcoming
pour at GB-1.
5. Offset pier at Ab3b.
o The Contractor was alerted to the engineer's requirement
that this pier and footing be removed and replaced in
it's proper location.

Observations
Reinforcing steel for GB-2,3,4, & 5 and B-1 has been placed.
Reinforcing rods in C-1 and C-2 will not be embedded in piers as
forms are currently laid out.
`.� Foundation wall at basement slab has been chipped in order to
free water stop from concrete wall.
Information
Contractor was informed and advised to check layout of caissons,
grade beams and reinforcing rods, identify discrepancy and
advise the architect of the situations.
Ralph Camerlingo was advised of Architect's concern for mis-
alignment of bolts and specification prohibiting burning of bolt
holes.
Ralph was also alerted to requirement for removal of pier and
footing at Ab,3b.

Weather: Overcast and drizzle; Temp. 70-75 degrees +/-
Work in Progress:
o Backfilling final lift in basement with borrowed gravel.
Material was not in compliance with Specification with stones
up to 6" diameter. Being compacted into the lift.
o Pier Ab3b has not yet been replaced.
o Concrete is being placed at GB-1,2,3,12,13,14, and beams B-3
and B-4.
o Washers were not being placed at the bottom of anchor bolts.
See Anchor Bolt and Base Plate Details on Drawing S-5.
o Water stop was being grouted into place using washed down
concrete from the shoot: Material was being taken from piles
of concrete left on the adjacent ground.
o Backfilling was done against grade beams placed last week.
Material was full of rather large stones up to 12" in
diameter.
o Condition of rods at C-1 and C-2 was not visible.
John Hansbury reports that when the steel and forms were set
in their final location all steel was contained in the grade
beams.
o Conditions at corner at C-4 and beam pocket at B-1 were not
visible. Unable to determine status of rebar/wall alignment.
Information
Received Proctor Density Test from The Welch Group Compaction
Test Data has not yet been received.
Bob Buthmann reports Compaction Test was done in A.M.

No concrete or soils inspector was present at this site at any
point during my visit.
Bob Buthmann reports project is on schedule and steel should be
delivered next week.
