Dear Mr. Young:
Due to a complaint received through the Neighborhood Improvement
Committee hot line, I conducted an inspection of the above mentioned property
and found the following violations:
1. Siding missing on front of building allowing entry to small
animals. (Please repair)
2. Gutters need immediate attention.
Please notify this department within fifteen (15) days upon receipt of
this letter as to what course of action you will take to rectify these
violations. Failure to do so will result in legal action being taken against
you.

On inspection of your property at 8, 8'k, and 8A Herbert Street, Salem
we found that an unsafe condition exist at the rear of 8 (specifically
8A) Herbert Street on the third floor.
The large beams holding the roof up are in poor shope and are at best,
supported somewhat questionable. This area should be shored up and braced.
An unsafe or danger sign will be affixed both to the outside entry and
the area upstaris in question.
1 You are hereby ordered to commence the needed repairs by noon time the
following day of receipt of this notice.

Our office received a complaint regarding your property located at 8 Herbert Street,regarding
unsafe conditions and various building code violations.
The most concerning off these defilements that has been received and noted were life safety and
egress stairway violations at the property,for these reasons an inspection must be conducted by
our inspection team to assure compliance with the code and city ordinances.
Under the provisions of 780 CMR, Section 116.3, the State Building Code, access to this property
must be granted for the purposes of this inspection. Please call this office upon receipt of this letter
to schedule this required inspection. If this property has rental units,these tenants must be notified
in advance of this inspection, so that access to these spaces may also be accomplished.
This inspection shall be on May 16,2013 at 2:00 p.m.; failure to respond to this notification will
be construed as non- compliance, and as such an Administrative Search Warrant will be sought, so
as to allow the lawful inspection of this property.
