Dear Property Owner;
Health and Building Department employees recently entered the building at 33 Hazel Street
in response to a burst pipe. At that time we noticed that work permitted in November 2007
has not been completed and that there is not adequate fire separation between units and
between units and exit stairs.
The City of Salem Building Department is revoking the occupancy permit for this
building, effective immediately.
No Occupancy permit will be issued until such time as work is completed and inspected by
this office.
You have the right to appeal this decision to the Massachusetts Board of Building
Regulations and Standards at 1 Ashburton Place Boston, Ma

2 . Asphalt shingles must be maintained or replaced .
3 . Wild animals are entering and living inside garage
( squirrels , raccoons ) .
4 . Repair all soffitts and facia .
5 . Tarp must be removed from garage extension and proper

6 . Siding must be completezt at main structure .
7 . Remove tarp and complete roof with proper materials on
main structure addition.
Please notify this department within fifteen ( 15 )
days upon receipt of this letter, to inform us as to what
course of action you will take to rectify these
