Mr. Ortiz Please be advised, City Electrician John J. Giardi was
asked to conduct an investigation by your tenant on the first floor. It was in
reference to concern about electrical safety issues. One outlet in the North
Bedroom was broken and shall be removed. There are also exposed wires
that have to be concealed. A new outlet shall be installed. There was one
broken outlet in the South side bedroom that was broken and one outlet was
also loose in the same bedroom. A licensed electrician is required to file an
electrical permit with the City of Salem, Mass. The work is to commence
upon receipt of this letter. The work is required to be completed within 7
days. Failure to comply with this request will result in Court Action. If you
have any questions please feel free to call me at my office.

Please be advised I have investigated the use of the above-entitled
real estate and based upon the information furnished me, it is my opinion
that it was a lawful three-family use prior to adoption of the Zoning
Ordinance on August 27, 1965. Accordingly, it should be afforded the
protection of Section VIII (E.) of the aforementioned ordinance relative

Our office received a complaint regarding your property located at 14 Harrison Avenue alleging
various building code and City ordinance violations. The most egregious of these violations is the,
possible operation of a rooming house in a Residential District. (See Regulations below)
Rooming House Regulations (from Salem Zoning Ordinance)
1. Rooming Houses are not allowed anywhere in the City except in BI
and B2 Zones.(refer to Zoning Map for which zone your building is in)
Seepage 6 of Zoning Code attached
2. Two-family or Multi family dwelling units are defined on page 83 of
the zoning code attached. A landlord cannot rent a dwelling unit to
more than 3 unrelated people or to any number of people not "living
together as a single non-profit housekeeping unit"
Per Mass State Building Code 780 C.M.R. Section R 104.1 and under the provisions of 780 CMR,
Section 104.6—Right of Entry, of the State Building Code, access to this property must be granted for
the purposes of this inspection. If this property has rental units, these tenants must be notified in
advance of this inspection, so that access to these spaces may also be accomplished. As per our phone
conversation;
Required Inspection shall be conducted by this office on Thursday, May 16, 2013 at 10:00 a.m.

The current exemption of "homeowners" was extended to include owner-occupied
dwellings of six units or less and to allow such homeowners to engage an in-
dividual for hire who does not possess a license, rovided that the owner
acts as supervisor. (State Building Code Section 109.1 .1

am providing you with the list of items which should be corrected in the porch
repair work:
- permanent front handrail mounts will be installed
any cracked or.bowed pads and bases will be, replaced
- all angled timber should be properly mitered
- all rotted decking should be replaced
- all lumber should be weather-sealed or primed
- old latticework should be hauled away
overall porch support should be sound and sturdy
- replaced porch skirt board(s) should be flush with original(s)
- overall workmanship should be of a professional manner
Additionally, you are to obtain a Salem license and building permit. I will
gladly help you with this matter.
I appreciate our meeting today and feel confident that all parties involved
will be satisfied at the completion of the project.
