This Department has received a report from Salem Fire Department. The report calls out
several deficiencies including no keys to the knox box. Other problems mentioned were
the dryer hookups in the basement and fire alarm problems.
Under the authority of Massachusetts State Building Code 780 CMR, Section 103 an
inspection,of the entire building has been set for November 12 at 1:00 p.m. An inspector
from Health, Fire and Electrical will also be present. All areas of the building need to be
accessed by the inspectors. Thank you in advance for your anticipated cooperation in
this matter.

was conducted by representatives of the Health Department on August 12, 1985. The
following was noted:

1. Smoke and fire damage to walls, ceiling, and cabinets in kitchen
2. Two windows broken in kitchen
3. Stove disconnected due to possible damage
4. Smoke detectors in apartment not operating
First Floor - Rear
5. Room One - Extensive fire and smoke damage
6. Room Two - No damage
7. Room Three - No damage
8. Room Four - Smoke and soot damage
9 . Room Five - Smoke and soot damage
10. Room Six - No damage

Based on the above-mentioned the Health Department condemns Apartment 4 on the
Second Floor and Rooms 1 through 6 and the Common Areas on the First Floor.
Please submit in writng within seven (7) days to this office your intent to
remedy these situations.
Should you be aggrieved by this Condemnation Order, you have the right to re-
quest a hearing before the Board of Health. A request for said hearing must be
received in writing in the office of the Board of Health within seven (7) days of
receipt of this Order. At said hearing, you will be given an opportunity to be
heard and to present witness and documentary evidence as to why this Order should
be modified or withdrawn. You may be represented by an attorney. Please also
be informed that you have the right to inspect and obtain copies of all relevant

1 received the attached subpoena on Tuesday,April 13, 1999. It is addressed to the Keeper of
the Records of the City of Salem, and requests that City Official(s) attend a deposition regarding property
at 13 to 15 Harbor Street.
Since I am the keeper of the records of the City Clerk's Office and City Council only, this
deposition is being forwarded to you so that your department can also address this matter.
1 have been in contact with the attorney who sent this subpoena to me. I told him that I do not
have jurisdiction over the records of other departments, however I would forward a copy of this notice to..
each department stated in Schedule A(attached). I also inquired if I could file an affidavit on behalf of my
office, instead of appearing for the deposition. He said that would be fine. The attorney also said that all
the departments listed on Schedule A may file affidavits instead of appearing at the deposition.
