This letter is to confirm that on January 7, 2025, David Greenbaum, Health Agent for the
Salem Board of Health, and Elizabeth Gagakis, Senior Sanitarian for the Salem Board of
Health conducted an inspection of the above referenced property for a complaint of no
heat.
During this inspection I conducted a re-inspection of your unit to check for compliance
with the housing code order I.issued on October 3, 2024.
At that time, it was noted that the violations cited on October 3, 2024, had not been
corrected, and as of the date of this letter the owner has not notified the Salem Board of
Health that these violations have been corrected.
