Dear Sir:
In accordance with Chapter II of the State Sanitary Code, 105 CMR 410.000, Minimum
Standards of Fitness for Human Habitation, a recent re-inspection was conducted of your
property located at 19 Leavitt Street, conducted by Jeffrey Barosy, Sanitarian of the Salem
Board of Health on August 12, 2026. At that time the following items were still outstanding:
Basement 1:
- The Salem Board of Health has not received notice of pest control work or any pest
control reports pertaining to this dwelling unit. If pest control is being conducted,
please send records to the inspector.
- The washer and dryer in the basement have not been repaired.
During the re-inspection the inspector observed there was no power in the bedroom
adjacent to the pantry. Have the power restored to all outlets and electrical fixtures in
this bedroom.
During the inspection, the occupant informed the inspector he would be moving out of
the dwelling unit at the end of August 2026. Before this dwelling unit can be rented to a
new occupant, the property owner will have all remaining violations and new violations
corrected, have the dwelling unit re-inspected by the Salem Board of Health, and acquire
a Certificate of Fitness. While the dwelling unit is unoccupied, you have no time limit to
complete the corrections. Failure to correct all violations and acquire a Certificate of
Fitness prior to having a new occupant move into the dwelling unit will result in the
property owner being issued fines of$50.00 per day until the dwelling unit is brought into
compliance.
All other violations noted in the inspection report of April 2, 2026, have been corrected.
Thank you for your cooperation in this matter.
