VIOLATION NOTICE

Dear Mr. Silva;
The above listed property has been found to be in violation of the following State
Codes and/or City Ordinances:
780 CMR, State Building Code, Section 1023 & 1014, requires operational exit
signage and emergency lighting at all exits and eggressible routes.
780 CMR, State Building Code, Section 1014.7, requires a handrail to be within
a 30"reach from any point on the staircase. Currently this staircase has one side
handrail; where two and a center rail are required.
780 CMR, State Building Code, Section 920.2 Subsection 1, requires that the
Portable Fire Extinguisher be on a hanger and in plain view, with signage to firrther
facilitate it's visibility.
The most recent Fire Alarm Test Report must be forwarded to this office.
There are numerous !roles in the vinyl flooring in the kitchen area that need to
be repaired per the State sanitary Code. (Health Department will be inform of this
situation.)
The Occupancy for this space is for the I"floor, only, there is no allowed use
for the basement other than storage. No Public use far rehearsal space and/or other
such uses are allowed.
Said violations must begin to be corrected, repaired, and/or brought into
compliance within 7 days of your receipt of this notice. Failure to do so may result in
further actions being brought against you, up to and including the filing of complaints at
District Court.
