As a result of an inspection conducted at 2190 P.M. this date of the
above construction area, the following recommendations are made which
will be in the interest of fire prevention and required to comply
with the code prior to certification by this office.
1. It is to be noted for the record that this office was requested
at the Noon hour to make an inspection of this new restoration
area for an approval of same. While it is our policy due to a
lack of experienced specialists in this office, to request an
appointment for said inspections at least 48 hours in advance,
I did provide this inspection on short notice, because I was told
it would create a hardship on the developers if not done immediately.
and the work was completed.
2. Upon inspection I found the following work was not done as noted$
a. Sprinkler systems
1. No notice for proper test has been received from the installer.
This is a requirement of the various codes.
2. The plans were approved on Dec. 17, 1974, which it shallbe
noted was long after the construction began.
3. Upon inspection I find no electric bell in place for sprinkler
alarm notification. ( The water motor gong requirement has
been eliminated as a deviation rec_uest for aesthetic reasons. )
4. The fire department connection appears in proper order, but
no test could be madeq because of advance notice lacking and
it was found that an opening was still existing on this supply
line in the basement. This would have flodded the basement
if used.
3. The supply side pressure gage on the O. S, & Y valve was not
working because it lacked a plug on the piping.
6. No pressure gage is installed at the inspectors pipe on the
third floor. This shall be installed on the system side of
the valve to show the static pressure at the high p6int and
the residual pressure upon a flow test. This shall be a per-
manant part of the system.
7' In the center stairwell between the second and third floor,
one sprinkler head was installed at a point directly in the
path of the partition. This necessitated the sheet rock being
_gut out which left an opening in the wall and will also
make the head deficient in operation.
8. It is also to be noted that the sprinklers are to remain
behind the lowered ceiling tiles. This was not approved as
part of the installation by this office. The subsequent
samples submitted to this office latter in the afternoon

were manufactured by Preferred Plastics Inc. and the data
sheets attached did not show this product to have an under-
writer of factory mutual approval for use below sprinklers.
Upon further investigation in my office I can not find this
company or product listed as approved in my copies of the
Building Materials Directory, This material to be .acceptable
under sprinklerso must therefore be certified by some testing
labatory in writing for acceptance by this office,
3. It has been found that the stairways to the third floor are open
from the second floor and it is hereby requested of the building
inspector to check for proper classification of the stairwell doors.
$. As of this date certain areas have been occupied prior to approval.
i On the first floor a clothing shop has occupied one section.
On the second floor a suite has been occupied by an attorneys
office and also an architect and others.
5. Carpeting has been installed in the second floor corridors and
the offices as well as the Hardiman dress shop, without any
certification as to the flame spread and smoke contribution. .
Said certification shall be made by the installers of the carpeting
as required by code.
6. An appointment has been made with Mr. Vini Amanti to re-inspect
for approval the sprinkler systemo at 200 P.M. on Monday Feb.

7. The Boiler room door is off its hinges and no emergency lights exist.

This letter is a clearification of the July 27, 1987 Building Inspector's
Department correspondence (see enclosed) . On Friday July 24, 1987 Robert E.
Blenkhorn, Health Agent, and William Burke, R.S. , met with a member of the
building Inspector's Office for a site assessment of the airconditioning unit
located on Front Street. Mr. Blenkhorn stated that the noise was not ex-
cessive; however, the large fan was not operating during the time of ob-
servation.
The Health Department requests anon site meeting with you and any other
individuals you feel necessary to get the whole scope of the problem.
The Health Department appreciates your cooperation in this matter.

Enclosed please find a letter from Rona Associates
outlining their suggestions regarding structural
issues on Essex Court,,specifically column base
plate repairs , cold weather concreting and
structural steel inspection. These last two issues
are addressed in detail in the Specifications , under
Division 3 and 5 respectively.
As far as the base plate repairs are concerned,
these should be undertaken as soon as possible,
before further loads are applied, and ought to be
completed - subject to the approval of the Architect
and Engineer - before any concrete slabs are poured
on the Essex Street building.
Please forward your proposals for repair to me and
to Rona Associates as soon as possible . If you have
any further questions regarding these matters ,
please let me know.

3) Structural_,Steel Inspection: uality control of structural
steel is treated _qted in division 5 , section 015120 . More , 1
specifically, moment connections should be tested as per
Massachusetts Building Code requirements using ultrasonic
testing. Bolted connections should-be_tested according to
.the latest edition of the -AISC-Code as specified- in- the
chapter -entitled Strutural Joints Using ASTM A325-_or.

elevator #2 - unsuitable fill beneath has been chased out.
Concrete to be poured (Friday) to within 3 " of bottom of
footing and remaining cavity packed with grout.
2 . Well point system on around pump chamber in courtyard -
re-excavation to begin when water table has dropped
sufficiently (el . 2 '-0 " - 3 '-0 " ) . Elliot Steinberg to
supervise stabilizing of footing @ D-12 and resetting of pump
chamber to proper elevation.
3 . Steel erection: structural steel up to 5th level on Essex
St. building . Bar joists tack welded to level 2 , and metal
decking in place.
4 . Underslab drainage system - PVC piping being installed in
last portion of link building , to approximately P-line.
