Notice: if this rental unit is occupied by a child or children under the age of 6 years, it is the property owner's responsibility to notify tenants of lead related reports and tests, and to ensure that this unit complies fully with 105 CMR 460:000: Regulations for Lead Poisoning Prevention and Control. For further information or to request an inspection, please contact the Salem Board of Health at(978) 741-1800. You are hereby ORDERED to make a good-faith effort to correct the violations listed on the enclosed inspection report and to take all positive action.to prevent these violations from occurring again in the future. Failure on your part to comply within the time specified on the enclosed inspection report will result in a complaint being sought against you in Trial Court. Time for compliance begins with receipt of this Order. Should you be aggrieved by this Order, you have the right to request a hearing before the Board of Health. Violation(s) Based upon a tenant complaint,an inspection was conducted in accordance with Article II of the State Sanitary Code, 105 CMR 410.000. Upon inspection,the following were noted: 24-48 hours .550 1. Tenant had complaint of mice in the building.At the time of inspection, mouse droppings were noted behind the toilet in the bathroom, inside the bathroom cabinet and in the basement.Owner to hire services of a licensed exterminator to inspect and treat all units and common areas of the building for mice:treatments must continue until problem is abated.Any maintenance or repairs recommended to help with exclusion of mice must be completed. Please forward invoices for service to the Board of Health for our records.Owner to correct this violation within 24-48 hours. 7 days .235, .270(A) 2. The back entry door to the building is broken and being held closed by a fence gate latch. Door must be replaced or repaired so it is able to be opened,closed,and secured properly: every exterior door of any residence must be equipped with a working lock. The front entry door to the building is open and unable to be locked.The main entry door of any residence with three or more units must have an automatically closing door equipped with a lock with an electrically operated striker mechanism. Repair or replace the front entry door to the building so it is compliant with 105 CMR 410.270(A). Owner to correct these violations within 7 days. 7 days .500 3. The back entry door to the unit has a gap at the bottom between the door and the floor. Install a sweep so the door excludes pests and the elements. Owner to correct this violation within 7 days. 7 days .351 4. Covers to the baseboard heaters in daughter's bedroom are damaged and falling off the heaters. Repair or replace so the covers are in good condition and properly fitted. Owner to correct this violation within 7 days 7 days .351 5. Caulk around the edge of the bathtub is damaged and mildewed. Remove old caulk and replace so it is in good condition.Owner to correct this violation within 7 days. 7 days .351 6. Knob to turn oven on appears to not be working properly. Repair or replace the knob so the oven may be safely used.Owner to correct this violation within 7 days. 7 days .500 7. There is a gap in the comer of the small basement bulkhead door where light is visible to the outside. Replace door or otherwise seal so there are no gaps around this bulkhead to assist in the exclusion of rodents.Owner to correct this violation within 7 days. 7 days .500 8. There are several areas and doorways in the unit with holes around the doorframe at the floor, including inside son's and daughter's bedroom doors with steel wool stuffed in the holes.Any such holes throughout the apartment must be found and properly sealed to help in excluding mice. Owner to correct this violation within 7 days. 7 days .500 9. There are gaps around the pipes behind the toilet.Seal the holes to help in excluding mice.
