Specified Time Reg.#410. Violation(s)
Based upon a tenant complaint, an inspection was conducted in accordance with Article II of the
State Sanitary Code, 105 CMR 410.000. Upon inspection,the following were noted:
24-48 hours .550 1. Tenant had complaint of mice in the unit. At the time of inspection, droppings were
observed under the kitchen sink, behind a dresser in the primary bedroom, and on
the floor under the windows in the kitchen. Owner to hire the services of a license
exterminator to inspect all units and common areas and treat accordingly for mice;
services must continue until problem is abated. Please forward invoices for service
to the Board of Health for our records. Owner to correct this violation within 24-48
hours.
24-48 hours .330 2. The combination smoke and carbon monoxide detector in the living room did not
work when tested. Repair or replace this unit so it is in working condition at all
times. Owner to correct this violation within 24-48 hours.
24-48 hours .330 3. Smoke detector inside the second bedroom is missing from its bracket. Provide a
working smoke detector for this bedroom. Owner to correct this violation within 24-
48 hours.
24-48 hours .330 4. There are smoke alarms beeping in both the front and back common hallways.
Replace batteries and/or replace the alarms to ensure they are in working
condition. Owner to correct this violation within 24-48 hours.
7 days .500 5. There is a hole in the wall behind the microwave. Repair so the wall is intact and
free from holes. Owner to correct this violation within 7 days.
7 days .500 6. There is one back basement window that is broken. Repair window so it is intact
and in good condition. Owner to correct this violation within 7 days.
7 days .500 7. There is a front basement window with cardboard over a missing pane of glass.
Repair this window so it is intact and in good condition. Owner to correct this
violation within 7 days.
7 days .570(C) 8. There is a ripped up, moldy carpet being stored in the basement. The owner shall
be responsible for maintaining in a clean, sanitary, and safe condition free of refuse
or other filth or causes of sickness, any part of the residence which is used in
common by all occupants of the residence. Carpet must be removed and properly
disposed of. Owner to correct this violation within 7 days.
30 days City of Salem 9. There is no certificate of fitness on file for this unit. In accordance with Salem city
code of ordinance, owner must apply for and obtain a Certificate of Fitness for this unit and
ordinances, ensure all other units in building have valid Certificates of Fitness when the
sec. 2-705 above violations have been corrected.
