Dear Sir/Madam:
In accordance with Chapter II of the State Sanitary Code, 105 CMR 410.000, Minimum Standards of
Fitness for Human Habitation, a re-inspection of your property located at 73 School Street#1 was
conducted by Elizabeth Gagakis, Senior Sanitarian of the Salbm Board of Health on July 20, 2023.
At the time.of re-inspection, the following violations remain outstanding:
Caulk around the shower seams and edge-seams around the shower enclosure are still loose and
mildewy. Seal/caulk all around shower enclosure so seams are watertight.
10. Crack in ceiling of primary bedroom -at the time of re-inspection a section of the ceiling in this
edroom had fallen out where the crack was. Repair the ceiling so it is intact.
19. Cross metering-please send letter from electrician to the Board of Health to confirm that all cross
metering in unit/house has been remediated.
*There were several violations of screens missing in the unit; some windows have had screens screwed
in from the outside, which does not meet the requirements of the code. Provide and install properly
fitted screens in every window in this unit. -I

Based upon a tenant complaint, an inspection was conducted in accordance with Article II
of the State Sanitary Code, 105 CMR 410.000. Upon inspection, the following were noted:
7 days .351 1.• Tub/shower enclosure has damaged and mildewed caulk along seams and edge of
® the tub. Remove and replace caulk/grout so tub and shower are intact and in good
condition. Owner to correct this violation within 7 days.
7 days .351 2. Windowsill in bathroom is mildewed and damaged. Remove and replace windowsill
so window is intact and in good condition. Owner to correct this violation within 7

7 days .500 3. There are cracks in the ceiling over the shower. Investigate for any leaking and
Irepair if necessary; repair ceiling and re-paint so it is intact and in good condition.
Owner to correct this violation within 7 days.
7 days .351 4. There is chipping and peeling paint on the bathroom wall. Scrape and repaint all
./ affected areas so walls are intact and in good condition. Owner to correct this
violation within 7 days.
7 days .500 5. Window in bathroom does not stay up when opened. Repair window so it is able to
J open, stay open, and close properly and is in good working order. Owner to correct
v this violation within 7 days.
7 days .500 18. Window over kitchen sink does not stay up when opened. Repair window so it is
J able to open, stay open, and close properly and is in good working order. Owner to
correct this violation within 7 days.
7 days .354 19. Electrical outlet at the base of the stairs in the basement is wired to the first floor
electric meter; all common area fixtures and outlets must be wired to a house
meter. Owner to hire a licensed electrician to remove this outlet and any other
common fixtures from the first floor meter and ensure that all common fixtures and
outlets in the building are wired properly. Please forward invoices for service to the
C'w Board of Health for our records. Owner to correct this violation within 7 days.
24-48 hours .600 20. There is old siding along the right side of the house and an old toilet in front of the
/ house. Remove these items and store properly until collection: property must be
maintained in a clean, sanitary manner free from any trash or debris. Owner to
correct this violation within 24-48 hours.
