Notice: if this rental unit is occupied by a child or children under the age of 6 years, it is the.property owner's responsibility to notify tenants of lead related reports and tests, and to ensure that this unit complies fully with 105 CMR 460:000: Regulations for Lead Poisoning Prevention and Control. For further information or to request an inspection, please contact the Salem Board of Health at(978)741-1800. You are hereby ORDERED to make a good-faith effort to correct the violations listed on the enclosed inspection report and to take all positive action to prevent these violations from occurring again in the future. Failure on your part to comply within the time specified on the enclosed inspection report will result in a complaint being sought against you in Trial Court. Time for compliance begins with receipt of this Order. Should you be aggrieved by this Order, you have the right to request a hearing before the Board of Health. Violation(s) Based upon a tenant complaint, an inspection was conducted in accordance with Article II of the State Sanitary Code, 105 CMR 410.000. Upon inspection,the following were noted: 24-48 hours .180 1. Tenants had complaint of heat not working adequately.At the time of inspection,the two back bedrooms were recorded at 66 degrees F:temperatures must be maintained at a minimum of 68 degrees F between 7:00am and 11:00pm,and at a minimum of 64 degrees F between 11:01 pm and 6:59am. Inspect/service heating system and the vents in the unit ensure it is working properly to maintain the unit at the proper temperatures of at all times. Owner to correct this violation within 24-48 hours. 7 days .500 2. Kitchen has areas on the wall and ceiling where the paint is cracked/chipped and in general disrepair. Scrape and repaint any affected areas so walls and ceiling are intact and in good condition.Owner to correct this violation within 7 days. 7 days .500 3. Pantry off the kitchen has areas on the walls and ceiling of cracked/chipped paint. Scrape and repaint to remove mold and mildew, repaint the ceiling so it is in good condition,and assess ventilation in bathroom to ensure it is being used properly to prevent future mold and..mildew from building up. Repair so handle is secured and in working condition.Owner to correct this violation within 7 days. 7, days .235 7. Last bedroom is missing a cover for the light. Provide a cover or globe for this light.Owner to correct this violation within 7 days. 7 days .235 8. Last bedroom has cracked and peeling paint on the ceiling and walls. Electrical outlet beneath the left window in the last bedroom is loose and coming out of the wall. Hire services of a licensed electrician to repair and secure this outlet so it is working and in safe condition. Please forward invoice for service to the Board of Health for our records. There is one smoke detector in the front hall but no others in the unit. Provide and install working smoke detectors in the unit in accordance with applicable codes and fire regulations:please contact Fire Prevention at(978)745-7777 for placement questions. There is one carbon monoxide detector in the unit but there is not one within ten feet of each bedroom as required; provide additional carbon monoxide detectors to meet this requirement. Owner to hire the services of a licensed exterminator to inspect and treat the entire building for mice:all recommendations from the exterminator to help in elimination of mice must be followed by owner,and treatments/inspections by the exterminator must continue until mouse problem is eliminated.
