Notice: if this rental unit is occupied by a child or children under the age of 6 years, it is the property owner's responsibility to notify tenants of lead related reports and tests, and to ensure that this unit complies fully with 105 CMR 460:000: Regulations for Lead Poisoning Prevention and Control. You are hereby ORDERED to make a good-faith effort to correct the violations listed on the enclosed inspection report and to take all positive action to prevent these violations from occurring again in the future. Violationi s Based upon a tenant complaint,an inspection was conducted in accordance with Article II of the State Sanitary Code, 105 CMR 410.000. Upon inspection,the following were noted: 24-48 hours .550 1. Tenant had complaint of mice in the unit.At the time of inspection, mouse droppings were noted under the dishwasher.Owner to hire services of a licensed exterminator to inspect the entire building,write up an integrated pest management plan(IPM)and treat all units as often as necessary to eliminate the mice.All deficiencies found during pest inspections must be remedied to help exclude mice and other.pests. Outlet to the left of the kitchen sink is not working.Owner to hire services of a licensed electrician to repair outlet so it is in safe working condition:please forward invoices for service to the Board of Health for our records. There are stains and falling plaster on the ceiling in the room to the left of the stairs on the third floor. Investigate for source of leaks and repair:scrape/repaint and repair plaster so ceiling and wall are intact and in good condition free from stains or damage.Owner to correct this violation within 7 days. 7 days .551 6. Smaller bedroom on third floor has a tom window screen. Patch or replace screen so it is .intact and in good condition. Owner to correct this violation within 7 days. 24-48 hours .482 7 There was no carbon monoxide detector on the third floor. Provide a carbon monoxide detector within ten feet of every sleeping area.Owner to correct this violation within 24-48 hours. 7 days .500 8. Windows opposite the television in the living room do not open;the right-hand crank window was observed to be screwed shut.Windows must be repaired or replaced so they are properly installed and open and close properly.Owner to correct this violation within 7 days. 7 days .500 9. There is staining and damp wood indicating a leak above the bottom of the basement stairs. Investigate for source of leak and repair so leak ceases:repair/replace any rotted/affected wood in the floor/over the stairs so it is intact and in good condition.Owner to correct this violation within 7 days. 7 days .351 10. Hot water tank in the basement appears to have a leak at the connector on the tank.Hire services of licensed plumber to inspect and repair tank and pipes so there are no leaks. Owner to correct this violation within 7 days. 7 days .351 11. There is a tag on the tenant's furnace with no date. Hire services of a licensed plumber to check furnace to ensure it is in proper and safe working condition. Owner to correct this violation within 7 days. 7 days .351 12. It appears an outdoor electrical outlet was removed from the side of the house on the left of the bulkhead. At the time of inspection, it was noted that at least one basement light and the light in the common first floor hallway are on the second floor meter. Owner to hire services of a licensed electrician to inspect wiring in the house, remove these common fixtures from the second floor meter,and put all common area fixtures onto a house meter.
