Dear Sir/Madam:
In accordance with Chapter II of the State Sanitary Code, 105 CMR 410.000, Minimum
Standards of Fitness for Human Habitation, a recent re-inspection was conducted of your
property located at 4 First Street Unit 9101, conducted by Jeffrey Barosy, Sanitarian of the
Salem Board of Health on March 16, 2026. At that time the following items were still
outstanding:
Dwelling Unit:
- In both bedrooms, there are missing ceiling mounted smoke/carbon monoxide
detectors. The City of Salem Fire Department has been notified.
Exterior:
- The main entryway door for Building 9 has been repaired but damaged again,
preventing the door from closing properly.
Remaining violations are ordered to be corrected within seven (7) days of receipt of this
report. Failure to bring the remaining violations into compliance within this time will
result in the inspector filing a complaint against the property owner in the Northeast
Housing Court. Please correct the outstanding violations and contact this office to
schedule a final re-inspection of this apartment.
All other violations noted in the inspection report of December 5, 2025, have been
corrected.

Observations:The temperatures in the dwelling unit's rooms at the time of inspection are as follows:

- Kitchen: 62.5*F

All temperatures were measured between 10:30am and 11:00am at the time of inspection.
At the time of inspection, the inspector observed the thermostat was turned to 70.0*F, but read 61.0*F. Repair the heating
system so during the Heating Season it can provide heat at a temperature between 68.0*F and 78.0*F between the times
of 7:00am and 11:00pm, and 64.0*F and 78.0*F between the times of 11:01pm and 6:59am.
Code Description: (A)The owner shall provide heat in every habitable room and every room containing a toilet, shower,
or bathtub from September 15th through May 31 st inclusive so that it shall be: (1)At least 68°F (20°C)between 7:00 A.M.
and 11:00 P.M

Observations:The main entryway door for Building 9,where the dwelling unit is located, has cracks in its doorframe near
the top door hinge.This damage prevents the door from closing. Repair the door frame so it is intact and free of defects,
and allows the door to open and close with ease.
