The above referenced property has come to the attention of this department for the
following reason(s):
A report has been made to this office that there is an illegal basement apartment
unit in the building. For this reason an inspection must be conducted by our inspection
team to assure compliance with the code and city ordinance.
Under the provisions of 780 CMR, Section 115.6, the State Building Code, access
to this property must be granted for the purposes of this inspection. Please call this office
upon receipt of this letter to schedule this required inspection. If this property has rental
units, these tenants must be notified in advance of this inspection, so that access to these
spaces may also be accomplished.
This inspection must be completed on or before October 7, 2008; failure to respond
to this notification will be construed as non- compliance, and as such an Administrative
Search Warrant will be sought, so as to allow the lawful inspection of this property.

It will be necessary for me to refuse to issue a permit for the
alteration of a store located at 155 Fort Ave. to provide for
an additional apartment, as this is a B-2 District, zoned for
Highway Busi4iesi .r and residential occupancy is non-conforinirig"£or
the district.
However, I wish to advise you of your right to appeal from this
decision to the Board of Appeal, who may in some cases grant a
variance from the application of the Zoning Ordinance.
The next meeting of the Board will be held on July 29, 1974•
If you wish to be heard at that time all pertinent data must
sm
be in by Jul: n'11" 1974.
We are enclosing forms to be filled out in triplicate, and the
rules for filing an appeal. The appeal forms , plot plans, and
working plans, aceompani6d by a check in the amount of $26.25
payable to The Salem News must be received by July 5, 1974 to
be scheduled for the July meeting.

THE HEALTH DEPARTMENT,FIRE PREVENTION,AND MARK ROCHON CITY OF SALEM
WIRE INSPECTOR WHERE REQUESTED TO INVESTIGATE A WATER DAMAGED
RECEPTACLE OUTLET IN UNIT 1. THE WALL WAS ROTTED AND THE OUTLET WAS
FALLING OUT OF THE WALL. THE RECEPTACLE OUTLET WAS REMOVED AND THE
CABLE WAS WIRE NUTTED,OWNER DONALD MOORE WAS PRESENT.
THIS OFFICE HAS NOT RECEIVED AN ELECTRICAL PERMIT FOR THE REPAIRS,
PLEASE TAKE TIM NECESSARY STEPS TO REPAIR THESE,ELECTRICAL HAZARDS. THIS
WORK SHALL BE DONE BY A LICENSED ELECTRICIAN WITH A PERMIT FROM THIS
OFFICE.
