Progress Report
Brief Description of Work This Week
The Contractor excavated blasted rock and overburden at the future sites for the Chlorine Contact
Chamber, Oxygen Reactor Area and Effluent Pump Station. The Contractor loaded hired trucks
with the product for offsite removal.
The Contractor completed excavation for the new duct bank which will run from the Effluent Pump
Station area to the Air Release Building.
The Contrauor-coinpieted excavation and installed ut;?;ties for the relocated the District.Engineering
trailers at the NEPCO staging area.
The Contractor assisted subcontractors SWP and Stocks, with rigging and excavation.
Stocks Blasting continued production and presplit blasting along the 66" plant effluent line at the
proposed Chlorine Contact Chamber and Effluent Pump Station. Stocks also continued to hoe-ram
small protrusions along the excavation perimeter and bottom.
Spencer White and Prentis completed installation of tiebacks 2A, 17, 20, 22, 23, 35A, and 37A.
They also worked proof testing installed tiebacks #34 and 35. Lagging was installed by SWP
between soldier piles at various locations. SWP bolted reinforcement dowels along the north wall
stabilization area and installed chain containment mesh at the same locations.
Annese Electric was onsite and completed electric feeders and telephone lines to the relocated SESD
Engineering trailers. They also worked installing conduit for the duct bank run to the Air Release
Building.
General Comments
Spencer, White, and Prentis has encountered extensive down time due to equipment failure.

Dear Mr. McCarthy:
At a regular meeting of the City Council held in the Council
Chamber on Wednesday, May 11, 1994, the Council voted to grant you
permission to move a Wood Frame house located at 53 Fort Avenue
subject to the following conditions :
1 . All necessary permits and permissions must be obtained
from the Electric Co. , Telephone, Co. , Cable T. V. Co. , Shade and
Tree Department, Street Department, Mass . State Department of
Public Works, and any other agents that may have an interest.
2 . Copies of Insurance Certificates must be presented, filed
in the City Clerk' s Office and must contain hold harmless clauses
that will absolve the city from any and all liability.
3 . On the day of the move all of the City Departments must
be notified. i .e. - Fire, Electric, Building, Police, Board of
Health, etc.
4 . A copy of a Purchase and Sales Agreement must also be
presented to the Building Inspector.

Salem Fire Prevention Regulation #29C
DEMOLITION OPERATIONS
In accordance with the provisions of the General Laws of the Commonwealth
of Massachusetts and the Salem Fire Code, the following regulation
governing the demolition of structures within the City of Salem shall be
followed.
This regulation is made in the interest of fire prevention, to prevent
conditions which are or may become dangerous as a fire hazard, or may
otherwise be in violation of law.
1. Before a structure can be demolished, the Salem Fire Department
requires the filing of an application for permit form #29C, and
shall conduct a preliminary demolition inspection to ascertain
any conditions which may require a fire watch or other preventive
measures to be taken before, during and/or after said demolition
process.
2. A permit to demolish or remove a building or structure shall not
be issued until said inspection has been conducyed, and any
requirements necessary for prevention of fire and life safety
hazards shall be shown on the permit, which bmust be shown to the
Building Inspector.
3. The requirements outlined in the permit shall become a part of
the restrictions portion of the demolition permit issued by the
Building Inspector, for compliance with the Code.
4. Tha permit may include, but not necessarily be restricted to the
following:
(a) Welding and cutting permit required.
(b) Fire watch to be maintained by a professional off-duty fire
fighter with such equipment as determined by the Head of the
Fire Department.
