The following violations of Article Two of the Sanitary Code of the Commonwealth
are brought to your attention:
1 st floor
1. Glass in front door is missing.
2nd floor - Doherty apartment
2. Holes in pantry ceiling, and leak from 3rd floor.
3. Bathroom has large hole in ceiling and also has leaks.
4. Provide electrical outlets in living room.
* 5. Front porch is in poor repair (both railing and floor weak)
(referred to Building Inspector) .
3rd floor
6. Front hall not maintained in a clean condition.
3rd floor - Soucy apartment
7. Toilet leaks when flushed.
8. Accumulation of rubbish on front porch.
9. Bathroom ceiling in poor repair (paint peeling.) .
It is also brought to your attention that as the owner of this multiple dwelling
you must provide an adequate number of rubbish containers for tenants, and also
provide adequate heat (70 degrees from 7:00 a.m. to 11:00 p.m.) and not less than
65 degrees from 11:00 p.m. 'to 7:00 a.m.).

after receipt of this notice.

rubbish containers with lids for use by occupants of this
multiple duelling.
A review of our files indicates legal action was necessary
previously to have corrective action in order to have your
property comply with the Sanitary Code of the Commonwealth of
Massachusetts.
Failure on your part to comply within 48 hours in the correction
of this nuisance condition will result in a complaint being filed
in the Salem District Court.
