Dear Sir:
An inspection of your property was made on Sept. 8, 1986 by this
department and the following items were found to be in violation.
Front and Rear Porches are in dangerous disrepair. Other obvious
deterioration exist on the exterior. Smoke Detectors are non-
functional. (Fire Dept. notified of this report.) Ballater and
Handrails in exitways need repair.
Storage of Motor Cycles in rear hall obstructs exitway.
Stairs to rear porch are in total disrepair, these must be rebuilt
immediately.
You are hereby ordered by this letter to acquire the needed permits
e and commence work within 24 hours of receipt of this notice. Failure
to comply will result—in legal action being brought against you.

Dear Sir/Dear Madam:
In accordance with Chapter 111, Sections 127A and 127B, of the Massachusetts
General Laws, 105 CMR 400.000: State Sanitary Code, Chapter 1: General
Administrative Procedures and 105 CMR 410.000: State Sanitary Code, Chapter
II: Minimum Standards of Fitness for Human Habitation, an inspection was
made of your property at 16 Forest Avenue Apt . 2 Salem, Massachusetts,
occupied by Karen King & Maureen Dumas This inspection was
conducted by V. Moustakis/Maureen Dumas Salem Health Department, on
9/8/86 at 10: 15 A.M .
Based upon said inspection, you are hereby ordered to take the following
action within 24 hours of receipt of this order:
Hot water had been restored to apartment. Hot water tank in cellar has
piping that appears questionable because it is not soldered together to
form solid piping and must be checked by Boston Gas immediately (Contact
Municipal Gas Inspector) . Other violations were noted as follows :
First floor apartment which tenants state is vacant must be secured to prevent
unlawful entry and possible fire hazard.
Must provide front hall first floor with a light bulb.
Back Hallways - Must remove all obstruction from both front and back hallways
o an exit is possible at all times .
V Back porches are in disrepair - 3rd Floor missing floor boards - Must clean
Y0 orches which are dirty and unsanitary because of pigeons.
, / Must repair other structural elements at roof on front and back porches .
Must repair and replace protective railing between 2nd and 3rd floors .
Must provide all hallways with lights (must provide bulbs) .

Based upon said inspection, you are hereby ordered to take the following

Must provide light bulbs in cellar at all times.
Must check cellar smoke detector to make sure it is operable.
Must repair back stairtread which is broken - this is a safety hazard -
Contact Building Inspector.
Must repair hallway smoke detectors which appear to be inoperable -
Contact Fire Department.
Based upon said inspection, you are hereby ordered to take the following
action within 3 days of receipt of this order :
Must scrape hallway ceiling which is flaking.
Based upon said inspection, you are hereby ordered to take the following
action within 5 days of receipt of this order:
Front stairs must be provided with protective left hand railing/and
must replace missing ballusters.
t./Front porch - Must replace splitting floor board which is a trip hazard.
Must repair front main entry door so it closes and opens properly and must
be made weathertight.
Front door does not have operable lock which does not have to be locked -
Must be provided with a lock.
Must repair/replace electrical outlet at 2nd floor with a coverplate.
Must provide back door with a locking mechanism.
There are no rodent proof watertight rubbish receptacles with tight fitting
lids in sufficient quantities for all tenants which must be provided.
Must clean exterior back yard.
Must seal hole in foundation of house (driveway)

All ceilings and walls and stairwells in hallways must be cleaned of all
accumulation of grime and swept and cleaned.

action within 30 days of receipt of this order :
Must replace missing bal lusters in the front stairway leading to second floor.

SAFETY AND WELL-BEING OF THE OCCUPANTS.
Failure on your part to comply within the specified time will result in a complaint

Should you be aggrieved by this Order, you have the right to request a hearing before
the Board of Health. A request for said hearing must be received in writing in the
office of the Board of Health within seven (7) days of receipt of this Order. At
said hearing, you will be given an opportunity to be heard and to present witness
and documentary evidence as to why this Order should be modified or withdrawn. You
may be represented by an attorney. Please also be informed that you have the right
to inspect and obtain copies of all relevant- inspection or investigation reports,
orders and other documentary information in the possession of this Board, and that
any adverse party has the right to be present at the hearing.
Please be advised that the conditions noted may enable the occupant(s) to use one
or more of the statutory remedies available to them as outlined in the enclosed
inspection report form.

` 1. One steam boiler, with oil burner, has been found unsafe for furthur
fir use. This unit has a Lynn oil burner, which was never approved and ,
as of this date is an illegal installation. This unit is for the
second floor and shall not be used until all permits are taken out
for same if a Certified Oil Burner Technician will agree to sign
for this unit.
2. The second older unit containing a I:ynn Oil Burner is also installed
i• without a permit for same. This unit shall not be used after
March 21 ; 1977 unless proper perwits are issued for same.
i; 3. The third unit, a new Carlin burner in a new boiler, was apparently
installed without a permit being taken out for the instal-lation.
f This. unit will also be illegal to use after March 21, 1977, unless
;)l proper,^permits are issued for same.
4. It is also noted that this being a three family dwelling unit, the
' boilerstshall be required to be enclosed in an approved boiler room. -
