Station Road Extension I hereby certify that, pursuant to Massachusetts General Laws, and the Subdivision Regulations of the City of Salem, the Salem Planning Board has taken the following action with respect to the above reference subdivision. By a vote of seven in favor, none opposed, the Planning Board approved the proposed plan as presented with attending conditions. Notice of the Board's action and the reasons supporting it are more fully described in the attached copy of a letter, the original of which was sent to the applicant by registered mail at his address stated on the petition. Power, III ��b Chairman M36WP of Salent, Aassac4usetts Planning Pnarl line Oulem Breen May 6, 1988 Mr. Paul DiBiase 10 Lookout Terrace Lynnfield, MA Dear Mr. DiBiase: The Salem Planning Board voted on May 5, 1988, to approve the proposed subdivision plan entitled, "Definitive Subdivision Plan, Station Road Extension," dated February 29, 1988, revised May 3, 1988, subject to the following conditions: 1. Conformance with Plan Work shall conform to plan entitled "Definitive Subdivision Plan, Station Road Extension," dated February 29, 1988, revised May 3, 1988. 2. Road Construction Road construction shall consist of 134" of wearing surface, 2" base over 12" of gravel. 3. Utilities Utility installation shall be reviewed and approved by the City Engineer, with a written report submitted to the Planning Board, prior to the issuance of building permits. 4. Board of Health Board of Health requirements set forth in a letter dated April 13, 1988, shall be strictly adhered to. 5. Conservation Commission The developer shall receive all necessary approvals of the Conservation Commission prior to the issuance of building permits. 6. Performance Guarantee A performance guarantee shall be filed with the Planning Board prior to endorsement of the plan to ensure completion of improvements required under the City of Salem Subdivision Regulations, and as required by this approval. 7. Trees a.Trees shall be a minimum of 334" caliper, planted 30 feet on center. b.Maintenance of landscape vegetation shall be the responsibility of the developer, his successors or assigns. r 8. Clerk of the Works A Clerk of the Works shall be provided at the expense of the developer as is deemed necessary by the City Planner. 9. Construction Practices All construction shall be carried out in accordance with the following conditions: a. No work shall commence before 7:00 a.m. on weekdays and 8:00 am. on Saturdays. No work shall continue beyond 5:00 p.m. There shall be no work conducted on Sundays or holidays. Inside work of a quiet nature may be permitted at other times. b. All reasonable action shall be taken to minimize the negative effects of construction on a), -tters. Advance notice shall be provided to all abutters in writing at least 72 hours prior to commencement of construction. c.Drilling and blasting shall be limited to Monday - Friday between 8:00 a.m. and 5:00 p.m. There shall be no drilling or blasting on Saturdays, Sundays or holidays. d. All construction vehicles shall be cleaned prior to leaving the site so that they do not leave dirt and/or debris on roadways as they leave the site. e All construction shall be carried out in accordance with the Rules and Regulations of the Planning Board. 10. As-Built Plans As-built plans, stamped by a Registered Professional Engineer or Land Surveyor, shall be submitted to the Planning Department and Engineering Department prior to issuance of Certificates of Occupancy. 11. Cleaning of Culvert Developer shall clean-out the culvert on Station Road on a yearly basis, as long as access is allowed across property owned by Raymond Beaulieu. 12. 15 Bradley Road Developer shall submit plans for the utility installation near 15 Bradley Road for the review and approval of the City Engineer and City Planner. 13. Violations Violations of any conditions shall result in revocation of this permit by the Planning Board. This endorsement shall not take effect until a copy of this decision bearing the certification of the City Clerk that twenty days have elapsed and no appeal has been filed, or that if such appeal has been filed, and it has been dismissed or denied, is recorded in the Essex County Registry of Deeds and is indexed in the grantor index under the name of the owner of record or is recorded and noted on the owner's Certificate of Title.
