The proposed Lot 23 set forth on the above-noted Subdivision Plan
contains 15 , 012 square feet of land, plus or minus , and is bounded
Southwesterly by Salt Wall Lane with a frontage of 151 feet. Lot 23
is a portion of Cabot Farm owned by your petitioners , being an area of
about 26 acres and the largest privately owned open area in the City of
Salem. Lot 23 will be conveyed to a niece of the petitioners, Christine
Lutts and her husband, as a present for them to build their family home
in proximity to other family members .
It is requested that the petitioners be granted a waiver of the
following requirements of Subdivision Regulations as not being pertinent
or necessary to this subdivision:
1 . Design scales for storm drainage, water and sewer.
2 . Environmental impact statement.
3 . Sign advertising public hearing.
4 . Performance guarantee.
5 . . Minimum road width of 57 ' = 36 ' street, granite curbs, grass
strips , 5 ' sidewalks .
6. Underground utilities .

Dear Beth:
At the Board of Health meeting held on May 9, 1989, a motion was made and passed
to conditionally approve the Definitive Plan for Orne Street Extension, with a
list of the following conditions:
1 . The design proposal concur with the City Engineering Department and City
Plumbing Inspector for utility tie-ins for drinking water, sanitary waste
(sewage) , gas lines and/or other utility service.
2. Employ an acceptable method for dust and street cleaning control during
site construction and submit a copy of the method utilized to the Health
and Planning Departments in writing.
3. Prior to site start-up and also during site construction, employ a licensed
pest control firm for site evaluation and service with a copy of the service
invoice sent to the Health and Planning Departments.
4. All backfilling be done in accordance with good engineering practices to
prevent future damage to all underground utilities. Only acceptable back
fill materials are to be used.

Dear Mr. & Mrs. Lutts
The Salem Planning Board voted on July 6, 1989, to approve the proposed subdivision plan
entitled "Subdivision Plan, Salt Wall Lane" dated March 23, 1989, subject to the following
conditions:
1. Conformance with Plan
Work shall conform to plan entitled "Subdivision Plan, Salt Wall Lane" dated March 23,
1989.
2. Utilities
Utility installation shall be reviewed and approved by the City Engineer.
3. Sidewalks/Planting Strip
A five foot sidewalk and a three foot planting strip shall be constructed along frontage of
lot.
4. Granite Curbing
6 inch vertical granite curbing shall be installed along frontage of lot.
5. Landscaping
A minimum 3 caliper tree shall be planted in the planting strip. The tree shall be
compatible with existing street trees on Salt Wall Lane.
6. Board of Health
Board of Health requirements set forth in a letter dated May 10, 1989, shall be strictly
adhered to.

7. Clerk of the Works
A Clerk of the Works shall be provided at the expense of the developer as is deemed
necessary by the City Planner.
8. Construction Practices
All construction shall be carried out in accordance with the following conditions:
a. No work shall commence before 7:00 a.m. on weekdays and 8:00 a.m. on Saturdays.
No work shall continue beyond 5:00 p.m. There shall be no work conducted on
Sundays or holidays. Inside work of a quiet nature may be permitted at other times.

b. All reasonable action shall be taken to minimize the negative effects of construction on
abutters. Advance notice shall be provided to all abutters in writing at least 72 hours
prior to commencement of construction.
c. Drilling and blasting and hoe ramming shall be limited to Monday - Friday between 8:00
a.m. and 5:00 p.m. There shall be no drilling or blasting on Saturdays, Sundays or holidays.
d. All construction vehicles shall be cleaned prior to leaving the site so that they do not
leave dirt and/or debris on roadways as they leave the site.
e. All construction shall be carried out in accordance with the Rules and Regulations of the
Planning Board.
9. As-Built Plans
As-built plans, stamped by a Registered Professional Engineer or Land Surveyor, shall be
submitted to the Planning Department and Engineering Department prior to issuance of
Certificates of Occupancy.
10.Violations
Violations of any conditions shall result in revocation of this permit by the Planning Board.
This endorsement shall not take effect until a copy of the decision bearing
certification of the City Clerk that twenty (20) days have elapsed and no appeal has been
filed, or that if such appeal has been filed, and it has been dismissed or denied, is recorded
in the Essex County Registry of Deeds and is indexed in the grantor index under the name
of the owner of record or is recorded and noted on the owner's certificate of title. The
fee for recording or registering shall be paid by owner or applicant.
I hereby certify that a copy of this decision and plans are on file with the City Clerk
and a copy is on file with the Planning Board.

Gentlemen:
We have reviewed the Rand Road subdivision for Carlton and Nancy Lotts and
offer the following comments which have been furnished to the owner.
1. A minimum clearance of 5-ft. should be maintained between the water
and sewer services.
2. Where cover is less than 3-ft. piping material should be ductile iron
or steel with concrete encasement.
3. Where cover is less than 4-ft. rigid insulation must be provided over
pipe for frost protection.
4. Invert at sewer manhole shall have channel formed to provide a smooth
transition of flow.

As discussed during the Board of Health meeting on June 10, 1986 the applicant
will submit to the following specifications:
1. The design proposal concurs with the City Engineering Department and City -
Plumbing Inspector for utility tie-ins for drinking water, sanitary waste
(sewage) , gas lines, and/or any other utility service.
2. Prior to water service (drinking water) being put into service it be tested
for pressure and bacteria by a certified laboratory and the results submitted
to the Planning and Health Departments in writing.
3. Employ an acceptable method for removal of all construction debris and
vegetation waste from site during construction.
4. Employ an acceptable method for dust and street cleaning control during
site construction and submit copy of method utilized to the Health Department
in writing.
5. Prior to site start-up and during site construction employ a licensed
pest control firm for site evaluation and service with a copy of the service
program sent to the Planning and Health Departments.
6. The final surface be graded so as to prevent any blockage or stagnant matter,
water, or organic growth which could create offensive conditions and/or odors.
7. All backfilling be done in accordance with good engineering practices to
prevent future damage to all underground utilities.

The proposed Lot 18 set forth on the above-noted Subdivision Plan
contains 26 , 601 square feet of land and is bounded Northerly by Way To
Cabot Farm (Sometimes known as Orne Street Extension ) and Southerly by a
50 ' easement of an extension to Rand Road as shown on said Plan . Lot 18 is
a portion of Cabot Farms owned by your petitioners , being an area of about
26 acres and the largest privately owned open area in the City of Salem .
Lot 18 will be conveyed to the son and daughter-in-law of Carlton G. and
Nancy B . Lutts as a present for them to build their family home in
proximity to other family members .
It is proposed , by reason of the family nature of this subdivis-
ion , that the 2001 of frontage on the Way to Cabot Farm be accepted ' as
proper frontage for this Lot . An expansive driveway opening off the Way of
sufficient width for the entrance and turning of fire equipment and other
vehicles will be constructed to the width directed by your Board .
It is requested that the petitioners be granted a waiver of the
following requirements of Subdivision Regulations as not being pertinent
or necessary to this subdivision :
1 . Design scales for storm drainage , water and sewer .
2. Enviromental impact statement .
3• Sign advertising public hearing.
4. Performance guarantee .
5. Minimum road width of 57 ' = 36 ' street , granite curbs , grass
strips , 5 ' sidewalks .
6. Underground utilities .

As a result of a request for an opinion from the Salem Fire Dept.
concerning the proposed development of a parcel of land, situated
on Orne St. Extension, and shown on a plan of land dated Dec. 27,
1983 as the property of Carlton G. and Nancy B. Lutts; the following
items are listed to provide our opinion on the matter.
1. Whereas this is the private land of one family, and is only the
third parcel of land to be developed, we will not consider this
project to add. a burden to the existing fire services required
in the area.
2. It shall be understood that any future development or sub-
division for prvdteuse of the remaining land would require
an updating of the services for the area.

I have reviewed the Subdivision Plan for the construction of a
single-family dwelling on Orne Street (Cabot Farm) and wish the following
to be placed on record:
The hydrant located within the bounds of Cabot Farm on Orne Street
(extension) has been flow-tested by the Public Works Department within the
past two days. According to the readings obtained by this department, a
fire flow of 750 gallons per minute (the ISO fire flow requirement for
this area) can easily be obtained from this hydrant.
The location of the proposed dwelling is served both by the hydrant within
the Cabot Farm bounds and another located on Felt Street at the
intersection of Orne Street.
In the future, should additional development take place beyond the
coverage of the most distant hydrant, consideration should be given to
improvements in the water system.

As a result of a request for an opinion from the Salem Fire Dept.
concerning the proposed development of a parcel of land, situated
on Orne St. Extension, and shown on a plan of land dated Dec. 27,
1983 as the property of Carlton G. and Nancy B. Luttsr the following
items are listed to provide our opinion on the matter.
1. Whereas this is the private land of one family, and is only the
third parcel of land to be developed, we will not consider this
project to add a burden to the existing fire services required
in the area.
2. It shall be understood that any future development or sub-
division for pr.ivateuse of the remaining land would require
an updating of the services for the area.
