At a regularly scheduled meeting of the Planning Board held on Thursday, March 17, 1994, the Board voted by a vote of six ( 6) in favor, none (0) opposed, to approve the application as complying with the requirements of a Special Permit, subject to conditions: 1 . Conformance with Plan Work shall conform to plan entitled "Definitive Subdivision Plan of Land Located in Salem, Mass. " two sheets; Sheet One dated March 13, 1992 with final revision dated February 1, 1994 and Sheet Two dated October 5, 1993, .. pr.epared by Eastern Land Survey Associates, Inc. ` 2 . Conservation Commission Work shall conform with all requirements of Order of Conditions issued by the Salem Conservation Commission on December 10, 1992 - DEP File #64-211. 3 . Board of Health Board of Health requirements set forth in a letter dated September 13, 1993, (from subdivision comment sheet) , shall be strictly adhered to. 4 . Construction Practices All construction shall be carried out in accordance with the following conditions: a. No work shall commence before 8:00 a.m. o w kd s , and Saturdays. Violations An'violation s of these conditions . shall result in revocation of this permit by the Planning Board. This Special Permit shall not take effect until a copy of the decision bearing the certification of the City Clerk that twenty days have elapsed and no appeal has been filed or that if such appeal has been filed, and it has been dismissed or denied, is recorded in the Essex County Registry of Deeds and is indexed in the grantor index under the name of the owner of record or is recorded and noted on the owner 's certificate of title.
