The Salem Planning Board voted on July 20, 2006 by a vote of nine (9) in favor and none
opposed to endorse "Approval Under Subdivision Control Law Not Required" on the following
described plan:
1. Applicant: Zbigniew Kantorosinski, 36 Forest Street & 19 Hershey Street, Salem, MA

2. Location: The parcel is located at 36 Forest Ave., assessors map 33, parcel 35.
3. Project Description: This Form A requested to divide a portion of 36 Forest Ave. into
two separate parcels. Parcel#1 having frontage on Hershey Street and consisting of
approx. 14,700 sq. ft., and parcel #2 having frontage on Forest Ave. and consisting of
approx. 12,414 sq. ft. Both parcels meet or exceed the required amount of frontage and
area and have adequate access on a public way.
