MASS �G Wne ,,'�alem Grern CLI IK'S 0FFICF I Decision Site Plan Review Special Permit Gardner Mattress c/o Attorney John Keilty 40 Lowell Street Peabody, MA 01960 RE: Gardner Mattress - 250-254 Canal Street On Thursday, December 5, 1996, the Planning Board of the City of Salem opened a public hearing regarding the application of Gardner Mattress, Inc., under Section 7-18, Site Plan Review of the Salem Zoning Ordinance, with respect to the construction of a 60 x 80 warehouse building and the demolition of existing storage trailers at Gardner Mattress, located at 250-254 Canal Street. The public hearing was closed on December 19, 1996. At a regularly scheduled meeting of the Planning Board held on December 19, 1996, the Board voted by a vote of six(6) in favor, none opposed , to approve the application as complying with the requirements of Section 7-18, subject to the following conditions: 1. Conformance with Plan All work shall conform with set of plans entitled, "Site Plan in Salem, MA", prepared for Gardner Mattress, prepared by Coastal Survey, dated July 8, 1996, revised October 23, 1996 and revised October 30, 1996. The first floor elevation of the proposed building shall be shown on the plan. 2. Amendments Any amendments to the above referenced plans or waiver of conditions contained herein may require the approval of-the Planning Board. 3. Utilities A. Utility installation shall be reviewed and approved by the City Engineer prior to the issuance of a Building Permit. B. All utilities shall be installed underground. 4. Board of Health d of Health requirements shall be strictly All Boar q y adhered to. 5. Fire Department All requirements of the Salem Fire Department shall be strictly adhered to. 6. Building Inspector All requirements of the Salem Building Inspector shall be strictly adhered to. 7. Landscaping A. A detailed landscaping plan and planting schedule shall be submitted to the City Planner and approved in writing prior to the issuance of a Certificate of Occupancy. B. Trees shall be a minimum of 3 ''/z' caliper. C. Pre-cast concrete curbing shall be installed within the parking area. D. The Applicant shall construct a fence or other approved screening along the rear property line, adjacent to the City's proposed bicycle trail. The design for such screening shall be reviewed and approved by the City's Landscape Architect prior to installation. 8. Sianaize Proposed signage shall be reviewed and approved in writing by the City Planner. 9. Conservation Commission All work shall comply with the Order of Conditions (464-260 ) issued by the Conservation Commission. ' 10. Clerk of Works The City's Clerk of Works shall be on site as deemed necessary by the City Planner. The cost for this service shall be borne by the applicant. 11. Maintenance A. The Applicant shall employ an acceptable method or means for the holding and disposal of trash after site development. A copy of this information shall be sent to the Planning Department and Health Department. B. Refuse removal, ground maintenance and snow removal shall be the responsibility of the applicant. C. Winter snow in excess of the snow storage areas on the site shall be removed off site. 12. Construction Practices All construction shall be performed in accordance with the Rules and Regulations of the Planning Board, and in accordance with any and all rules, regulations and ordinances of the City of Salem. 13. As-Built Plans As-built plans, stamped by a Registered Professional Engineer or Land Surveyor, shall be submitted to the Planning Department and the Engineering Department prior to the issuance of a Certificate of Occupancy. 14. Violations Violations of any condition contained herein may result in revocation of this permit by the Planning Board. I hereby certify that a copy of this decision and plans have been filed with the City Clerk and copies are on file with the Planning Board. The Special Permit shall not take effect until a copy of this decision bearing the certification of the City Clerk that twenty(20) days have elapsed and no appeal has been filed or that if such appeal has been filed, and it has been dismissed or denied, is recorded in the Essex South Registry of Deeds and is indexed under the name of the owner of record or is recorded on the owner's Certificate of Title.
